COVID-19 İntrauterin Fetal Ölüme Neden Olur mu? Olgu Sunumu

Özet
Koronovirüs pandemisinden dolayı gebelerin ve fetüslerin kötü perinatal sonuçlar açısından özellikle risk altında oldukları öne sürülmektedir. Ancak, koronavirüs hastalığının (COVID-19) abortus veya ölü doğum için yüksek riskle birlikte olup olmadığı bilinmemektedir. Bundan dolayı, bu çalışmada ikinci trimesterde ağır akut solunum sendromu koronavirüsü 2’nin (SARS-COV-2) enfeksiyonuna bağlı gelişen bir fetal ölüm olgusu sunulmaktadır. Yirmi yaşında bir gebe 22. gestasyonel haftasında başlıca pelvik ağrı şikayetiyle perinatoloji kliniğine başvurdu. Hastanın on bir gün önce gerçek zamanlı polimeraz zincir reaksiyonu (RT-PCR) kullanılarak yapılan nazofaringeal sürüntüsü SARS-COV-2 için pozitifti. Hastanın COVID-19 sırasında ateş ve eklem ağrısı gibi hafif semptomları ilaç kullanmadan gerilemişti. Hastanın obstetrik öyküsünde herhangi bir anormallik yoktu. Ultrason tarama sonuçları normal anatomi ve amnion mai ile birlikte intrauterin fetal ölümü ortaya çıkardı. Gebeliğin terminasyonu sonrası fetal otopsi yapıldı.
Abstract
It has been proposed that pregnant women and their fetuses may be particularly at risk for poor outcomes due to the coronavirus pandemic. However, it is not known whether coronavirus disease (COVID-19) is associated with a higher risk of miscarriage or stillbirth. Therefore, in this report, we present a case of fetal death during the second trimester due to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. A 20 years old pregnant was presented at 22 weeks of gestation to the perinatology department with a chief complaint of pelvic pain. Her nasopharyngeal swab was tested positive for SARS-COV-2 using real time polymerase chain reaction (RT-PCR) eleven days ago. Mild symptoms of patient such as fever and arthralgia were relieved without medication during COVID-19. Her past obstetric history was unremarkable. The results of ultrasound scan revealed intrauterine fetal demise with a normal fetal anatomy and amniotic fluid. The fetal autopsy was performed after termination of pregnancy.
