Subakut Tiroiditte İnflamatuar Markerlar Prognozu Öngörebilir mi?

Özet

Amaç Çalışmamızın amacı subakut tiroidit ( tanılı hastalarda kan parametrelerinden hipotiroidi gelişimini öngörecek markerlar elde etmektir Materyal Metod Çalışmaya 133 SAT tanılı hasta alındı Hastalar hipotiroidi gelişen ve gelişmeyen olarak iki gruba ayrıldı Her iki gruptaki hastaların hemogram albumin ve C reaktif protein ( seviyeleri ve bunlardan edilen trombosit albumin oranı PAO), trombosit CRP oranı PCO), CRP albumin oranı CAO) karşılaştırıldı Bulgular Hipotiroidi gelişen hasta grubunda serum trombosit düzeyi diğer gruptan istatistiksel anlamlı yüksekken, albümin ve CRP nin anlamlı düşük olduğu görüldü (sırasıyla p< 0 001 p< 0 001 ve p= 0 003 Trombosit düzeyinin yüksek, CRP ve albümin düzeylerinin düşük olması nedeniyle yapılan PAO, CAO, PCO gibi oransal değerlerde her iki grupta istatistiksel anlamlı fark saptandı (sırasıyla p< 0 001 p= 0 035 p< 0 001 ROC analizinde PAO 113 51 değerinin SAT tanılı hastalarda kalıcı hipotiroidiyi öngörmede 81 sensitivite ve 93 spesifiteye sahip olduğu, PCO için ise 9 06 değerinin 88 sensitivite ve 69 spesifiteye sahip olduğu görüldü Regresyon analizinde PAO nun kalıcı hipotiroidi gelişimini ön görmede bağımsız risk faktörü olduğu saptandı Sonuç Çalışmamız, SAT tanısında yaygın olarak kullanılan ve inflamasyon göstergesi olan PAO ve PCO nun hipotrioidi gelişimini öngörebileceğini göstermiştir Bu özelliği ile SAT tanılı hastalarda kalıcı hipotiroidi gelişimini predikte etmede bağımsız bir risk faktörünü koyan ilk çalışmadır ve literatürde benzer çalışma bulunmamaktadır Eğer tedavi seçimi gerçekten kalıcı hipotiroidi gelişimini etkiliyorsa hangi hastada hangi tedavinin seçileceği önemlidir Tedavi seçimine karar vermek için de tanıda bakılan PAO ve PCO değerleri bizi yönlendirebilir

Abstract

Objective The aim of our study is to obtain markers that will predict the development of hypothyroidism from blood parameters in patients with a diagnosis of subacute thyroiditis ( Material Method A total of 133 patients with SAT diagnosis were included in the study The patients were divided into two groups as hypothyroid and non hypothyroid Hemogram albumin and C reactive protein ( levels and platelet albumin ratio ( platelet CRP ratio ( CRP albumin ratio ( of the patients in both groups were compared Results While the serum platelet level was statistically significantly higher in the hypothyroid patient group than in the other group albumin and CRP were found to be significantly lower ( 0 001 p< 0 001 and p= 0 003 A statistically significant difference was found in both groups in values such as PAR, CAR, and PCR ( 0 001 p= 0 035 p< 0 001 In the ROC analysis, PAR 113.51 value was found to have 81% sensitivity and 93% specificity in predicting permanent hypothyroidism in patients with a diagnosis of SAT. In the regression analysis, it was determined that PAR was an independent risk factor in predicting the development of permanent hypothyroidism. Conclusion: Our study showed that PAR and PCR, which are indicators of inflammation, can predict the development of permanent hypothyroidism. If the choice of treatment really affects the development of permanent hypothyroidism, it is important which treatment to choose for which patient. PAR and PCR values can guide us to decide on the choice of treatment.
