Özet

Düzenli olarak hemodializ uygulanan kronik böbrek yetmezliği hastalarında arteriovenöz fistül (AVF) trombozu, morbiditeyi arttıran önemli bir komplikasyondur. Beş kez AVF operasyonu uygulanan ve son dört operasyon sonra¬sında h.eparine ceyapsızlık nedeniyle erken dönemde AVF'de tromboz gelişen kronik böbrek yetmezliği hastası İs¬tanbul Üniversitesi İstanbul Tıp Fakültesi Göğüs Kalp ve Damar Cerrahisi Anabilim Dalı'na sevk edildi. Hasta hiper-koagulabilite yönünden araştırıldı. Hematolojik tetkiklerinde Antitrombin III (AT III) düzeyi normalin %40'ı (N= %80 -%120) düzeyinde bulundu. Yeni bir AVF operasyonu öncesinde AT lllkonsantresinin intravenöz infüzyonu ile defi-sit giderildi. Lokal anestezi ile sol kolda daha önce yapılan radiosefalik AVF seviyesinin proksimaline yeni bir AVF operasyonu yapıldı. Operasyon sırasında heparinli serumun lokal intralumirtal kullanılması ile efektif antikoagulas-yon sağlandı. Postoperatif erken dönemde ve onsekiz aylık takibi boyunca AVF bölgesinde tromboz komplikasyonu görülmedi.

Abstract

Arteriovenous fistula (AVF) thrombosis is an important complication which increases the morbidity in patients with chronic renal failure undergoing regular hemodialysis. The patient with chronic renal failure that subjected to AVF operation for five times and experienced early thrombosis due to unresponsiveness to heparin in the last four procedures was refered to the Department of the Thoracic and Cardiovascular Surgery in İstanbul University, Fa¬culty of Medicine. He was investigated for hypercoagulability state. Haemotologic studies revealed that the Antith¬rombin III (AT III) level was 40% of normal (Normal range=80%-120%). The deficit was completed with the intrave¬nous infusion of AT III concentrate before the next AVF operation. A new AVF was recostructed proximal to the for¬mer radiocephalic AVF on the left forearm with local anesthesia. Effective anticoagulation was obtained by using lo¬cal intraluminal heparinized saline. No thrombotic complication was occured at the AVF site early postoperatively and a follow up period of eighteen months.
