Özet

Bu çalışma Leriche-Fontaine sınıflamasına göre evre III ve IV olup, cerrahi tedavi şansı olmayan periferik tı¬kayıcı arter hastalarının medikal tedavisinde pentoksifilin ve prostaglandin E1 tedavilerinin karşılaştırılması amaçlanmıştır. PGE1 günlük 20 ( gr. olarak intraarterial infüzyon yoluyla ve 3 hafta süreyle 11 hastaya uygu¬lanmıştır. Diğer gruptaki 11 hastaya da günde 1200 mg pentoksifilin intravenöz infüzyonla 10 gün süreyle verilmiş, tedavi oral pentoksifilin preperatiarıyia aynı dozda 3 haftaya tamamlanmıştır. Ölçülen ayak/kol indeks¬leri her iki grupta anlamlı "olarak artmıştır. İki tedavi grubundaki hastalarda da istirahat ağrıları kaybolmuş, ülserler iyileşmiştir. Analjezik kullanma ihtiyacındaki azalmalar ve ağrısız yürüme mesafesi sonuçları bakımın¬dan gruplar arasında büyük farklılıklar yoktur. Her ne kadar PGE1 tedavisi biraz daha etkili görünüyorsa da, pentoksifilin tedavisinden 20 kat daha pahalı olması sebebiyle, PGE1 tedavisinin periferik tıkayıcı arter hastalıklarının tedavisinde tercih edilmesi gereken yöntem olduğunu söylemek zordur.

Abstract

This study was performed for the comparison of pentoxifylline and prostaglandin E1 in the conservative treat¬ment of patients in Leriche - Fontaine stage III and IV who had no chance of vascular surgery. Prostaglandin E1 was given 20 (gr daily to 11 patients by intraarterial infusion for 3 weeks. In another group of 11 patients intra¬venous pentoxifylline was given 1200 mg per a day in the first 10 days and same dose of pentoxifylline was applied orally until the treatment period of 3 weeks was completed. The results of ankle/ brachial index were inc¬reased significantly in two groups. Rest pain improved and ulcers healed completely either in two types of tre¬atment methods. There is no big difference in reduction of analgesic consumption and the and the results pain-free walking distances in two groups. Although PGE1 treatment seems a little bit more effective, it 20 times mo¬re expensive than pentoxifylline treatment so it's difficult to say that PGE1 treatment is appropriate as medical tre¬atment method for the peripheral arterial occlusive disease.
