Karotis endarterektomisi sırasında nörolojik morbiditeyi etkileyen faktörler

Aralık 1996 tarihine kadar, kliniğimizde karotis endarterektomisi uygulanan 162 olgudan 141'i (%87) semptoma-tikti ve bunların 52'sinde (%36.9) geçici iskemik atak, 89'unda (%63.1) minör strok saptanmıştı. 21 (%13) olgu ise asemptomatikti. Karotis endarterektomisi sırasında intraarteryel şant, internal karotis stump basıncına göre 61 (%37.8) olguda kullanıldı. Arteriotomi 129 (%79.7) olguda primer, 26 (%16) olguda patch plasti ile kapatıldı. Yedi (%4.3) olguda ise eversiyon endarterektomisi + kısaltma ve karotis interna reimplantasyonu yapıldı. Perioperatif strok 6 (%3.7) olguda gelişti. Strok nedeni 3 olguda peroperatuar embolizasyon, bir olguda karotis klampajı sırasında gelişen serebral iskemi, 2 olguda ise postoperatif erken dönemde ortaya çıkan internal karotis trombozudur. Hasta seçiminin de önemli bir etken olmasına rağmen, perioperatif strok genellikle cerrahi teknikteki önlenebilir hatalara bağlıdır. Bu hataların azaltılması ile, ekstrakranyal karotis arterlerinin hastalığına bağlı strokun önlenmesindeki etkinliği çeşitli çalışmalarla kanıtlanan karotis endarterektomisi, daha da geniş bir hasta spektrumunda uygulanabilen, güvenilir bir tedavi olarak yerini alacaktır.
Abstract
Of the 162 carotid endarterectomies performed for 141 symptomatic cases till December 1996, the indication was transient ischemic attacks in 52 (36.9%) and minor stroke in 89 (63.1%). Twenty-one (13%)of the case were asymptomatic. An intra-arterial shunt was used in 61 (37.8%) of the operations according to the internal carotid artery back pressure. Primary closure of the arteriotomy in 129 (79.7%) cases and patch angioplasty in 26 (16%) ca¬ses were the method of choice. Eversion endarterectomy + shortening and reimplantation of the internal carotid artery were performed in 7 (4.3%) cases with stenosis+kinking of the carotid artery. Thirty days perioperative stroke rate was 3.7% (6 cases). Cause of the stroke was peroperative embolization in 3 cases, cerebral ischemia during the carotid clamping in one case and thrombosis of the internal carotid artery early in the postoperative period in 2 cases. Although patient selection is an important factor in the perioperative stroke rates of the carotid endarterectomy, most of the perioperative strokes are due to preventable technical errors made during different stages of the operation. Therefore, knowledge of these errors and their precautions will provide further decrease of the perioperative stroke rate.
