Özet

Periferik Parenteral beslenmenin (PPB) en önemli komplikasyonlarlndan olan infüzyon tromboflebitinin oluşmasında, vene ve katetere ait faktörler yanında, verilen sıvının pH'ı, osmolaritesi ve içeriği önemlidir. Bu klinik çalışmanın amacı, infüzyon tromboflebiti oluşmasında lipid içeren ve içermeyen sıvıların etkilerini araştırmaktır, iki gruba ayrılan hastalardan birinci gruptaki 15 hastaya glukoz, aminoasit, elektrolit içeren solüsyon, ikinci gruptaki 15 hastaya" ise glukoz, aminoasit, elektrolit ve lipit içeren solüsyon verildi, infüzyon bölgeleri tromboflebit açısından değerlendirilerek, Maddox sınıflamasına göre skorlandı. ikinci gruptaki tromboflebitlerin skor ortalamaları anlamlı olarak daha düşüktü (p< 0.05). Birinci grupta ise 3 hastada tromboflebit nedeni PPD sonlandırıldı. Lipid içeren parenteral beslenme solüsyonları pH ve osmolarite etkilerine de bağlı olmaksızın, periferik endoteli koruyarak tromboflebit oluşumunu önleyebilirler.

Abstract

Not only the factors belonging to veins and catheters, the pH, osmolarity and the composition of the solution are very important to prevent infusion thrombophlebitis which is one of the most important complications during peripheral pareriterall nutrition (PPN). The aim of this clinical study is to investigate the influence of the solution with or without lipid content to the Infusion thrombophlebitis. The patients were divided into two groups. The solutions with the contents of glucose, aminoacid, electrolyte were given to the first group which included 15 patients. To the second group of 15 patients the solution with the contents of glucose, aminoacid, electrolyte and lipid were given. The infusion sites were evaluated for the risk of thrombophlebitis and they were scored according to Maddox classification. Maddox score medians of the thrombophlebitis in group II patients are significantly lower than group I patients (p< 0.05). The PPN were abandoned in the 3 patients in the first group because of thrombophlebitis. The parenteral nutrition solutions with lipid content may prevent thrombophlebitis by protecting effects on the vascular endothelium of peripheral veins independent from the pH and osmolarity of the solutions.
