Multitravmatize hastalarda venöz tromboemboli profilaksisinde düşük molekül ağırlıklı heparin ( Enoksaparin ) kullanımının yeri

Şubat 1996 - Mayıs 1997 tarihleri arasında l.Ü. istanbul Tıp Fakültesi Acil Cerrahi Yoğun Bakım Ünitesi'nde teda-vi gören immobil multitravmatize hastalara kesin tanı konulduktan sonra ilk günde itibaren venöz tromboembo-li (VTE) profilaksisi için Enoksaparin sodyum (CLEXAN; Eczacıbaşı, Rhône-Poulenc) 40 mg/gün subkutan tekdoz olarak uygulanmıştır. En az 1 hafta boyunca hasatenede yatan 76 hasta çalışma kapsamına alınmış ve hastalar mobilize oluncaya kadar Clexan 40 mg/gün olarak devam etilmiştir. VTE risk faktörlerinden (yaş > 45 ve 3 gün-den fazla immobilizasyon, anamnezinde venöz tromboemboli varlığı, Claskow Koma Skalası > 7, kuadripleji / pa-rapleji, vertebra, pelvis veya alt ekstremite fraktürü olması, alt ekstremitede komplike yara veya majör ven yara-lanması varlığı) 1 veya daha fazlasını içeren hastalar yüksek riskli, risk faktörlerinden herhangi birini içermeyen hastalar düşük riskli olarak değerlendirilmiştir. Yüksek risk grubundaki 26 hastaya 7. gün rutin noniyonik kontrast madde (Omniopaque 300 mg) ile bilateral flebografi çekilmiş ve DVT araştırılmıştır. Klinik olarak PE (pulmoner emboli) şüphelenilen (taşikardi, taşipne, göğüs ağrısı) 6 hastaya akciğre ventilasyon-perfüzyon sintigrafisi yapıl-mıştır. Düşük risk grubundaki 50 hastadan 3'ünde klinik olarak DVT şüphelenilmiş (ekstremitede ağrı, duyarlılık, çap artması) ve bilateral flebografi çekilmiştir. Bu gruptaki hastalarda PE şüphesi olmamış ve buna yönelik tetkik yapılmamıştır. Hastaların hiç birinde DVT saptanmamış, 1 hastanın sintigrafisi şüpheli PE olarak değerlendirilmiş-tir. Clexan'a bağlı kanama komplikasyonu görülmemiştir. Sonuç olarak, multitravmatize hastalarda enoksapirin ile VTE profilaksisi gereklidir ve faydalıdır.
Abstract
Between February 1996 and May 1997, 40 mg Enoxaparin sodium (Clexan; Eczacıbaşı, Rhoune-Poulenc) was given subcutaneously once daily to the immobilized multiple trauma patients who were managed at the Inten-sive Care Unit of Emergency Surgery Department of Medical School of Istanbul University after their diagnoses were established. Seventy-six patients who stayed at the hospital at least 7 days were included in this study and Clexan administration (40 mg per day) was continued until the patients were mobilized. All trauma patients with one or more following risk factors for the development of VTE (age >45 years and enforced bed rest> 3 days; previous history of venous thromboembolism; Clascow Come Scale > 7; quadriplegia / paraplegia; spine, lower extremity or pelvis fractures; injury of a major lower extremity vein; complex wound of lower extremity) were assigned to the high-risk group. If none of the factors vwere present, the patient was assigned to the low-risk group. Twenty-six patients in high-risk group underwent bilateral contrast venograpy with nonionic contrast (Omniopaque 300 mg) to screen DVT at seventh day routinely after injury. Six patients suspected of having PE on clinical findings (tachycardia, tachypnea, chest pain) in high-risk group underwent ventilation perfusion scan-ning. Three of 50 patients suspected of having DVT based on clinical findings (pain, swelling, calf tenderness) in low-risk group underwent venography. There were no findings suspicious of PE in this group. None of the patients developed DVT, a ventilation perfusion scanning was assesed being as a suspected PE. Bleeding comp-lications due to the use of Clexan were not observed. As a result, the use of enoxaparin in preventing VTE in multiple trauma patients is effective and necessary.
