Özet

Renal arteriovenöz fistül, nefrektomi sonrası gelişebilecek nadir komplikasyonlardan biridir. Özellikle arter ve venin birlikte fiksasyon dikişi ile ligature edildiği olgularda daha sık görülen bu fistüller yüksek debili oldukları için zamanla konjestif kalp yetmezliği gibi kardiyovasküler patolojilere yol açabilirler. 40 yaşındaki kadın hastanın fizik muayene ve tetkiklerinde taşikardi, sol ventrikül hipertrofisi ve orta derecede hipertansiyon mevcuttu. Renal anjiografide sağ renal arter ile vena cava arasında arteriovenöz genişleme tespit edilmiştir (Resim 1). Kontrastlı bilgisayarlı tomografide (BT) ise anjiografiden farklı olarak sağ böbrek lojuna uyan bölgedeki anevrizmatik genişlemenin çevresinin kalsifik olduğu saptanmıştır. Aort ile vena cava inferior'un aynı fazda kontrast aldığı görülmektedir (Resim 2). Operasyonla renal arteri proksimalden ligature edilen hasta postoperatif 10. günde taburcu edilmiştir. Bu yazıda, literatürde nadir olan nefrektomi sonrası gelişen arteriovenöz fistül konusu referans kitaplar ve son literatür gözden geçirilerek ele alınmıştır.

Abstract

Renal arteriovenous fistula following nephrectomy is a rare complication. Although renal biopsy is the most common cause of renal arteriovenous fistula, the formation o a fistula between the renal artery and the renal vein following nephrectomy can be seen. The use of mass ligation of the renal pedicle,especially with suture ligatures, predisposes a patient to this complication. These fistulas are typically large, producing congestive heart failure and other cardiovascular manifestations. The patient was a 40-year-old woman having tachycardia, left ventricular hypertrophy and moderate hypertension. An arteriovenous fistula was seen at renal angiographie study (figure 1). On contrast computed tomography was seen that the aneurism was calcified (figure 2). We ligated right renal artery proximal to the aorta and discharged the patient on 10th postoperative day. In this study we presented this rare case related reference books and recent literature reviews.
