Özet

lliyak, femoral,popliteal ve distal alt ekstremite arterleri atherosklerotik lezyonlara bağlı sık olarak tıkanmasına rağmen aortanın akut tam oklüzyonu sık olarak rastlanılan bir patoloji değildir. Bu yazıda Ege Üniversitesi Kalp Damar Cerrahisi Kliniği'nde 1993-1998 yılları arasında akut total abdominal aort oklüzyonu nedeniyle ameliyat edilen 10 hasta etiyoloji, değişik klinik semptomlar, uygulanan tanı ve tedavi yöntemleri açısından incelenmiştir. Klodikasyo intermitant şikayeti infant hasta hariç tüm hastaların hikayesinde vardı. Akut olarak başlayan değişik derecelerde alt ekstremite iskemisi en sık hastaneye geliş semptomu, trombozis ise en sık akut aortik oklüzyona neden olan etiyolojik etmen idi. Arteriyal dolaşım 2 hastada embolektomi, 3 hastada embolektomi ve aksillo-bifemoral bypass, 3 hastada transabdominal aortik trombektomi ve aorta-femoral veya iliyak Y greft interpozisyonu,1 hastada direkt uygulanan aksillobifemoral bypass, 1 hastada ise torasik aortadan iliyak arterlere kadar konan Y greft interpozisyonu ve viseral arterlere yapılan endarterektomi ile sağlanmıştır. 10 hastanın 4'ü (%40) postoperatif dönemde şiddetli asidoz ve barsak iskemisi nedeniyle kaybedilmiştir. Sonuçlarımıza göre düşük ejeksiyon fraksiyonu, daha önce geçirilmiş olan miyokard enfarktüsü, parastezi ve paralizinin bulunduğu şiddetli alt ekstremite iskemisi, viseral arterlerin trombozisinin hastalarda kötü prognozun bir göstergesi olduğu saptanmıştır. Literatür ve tecrübelerimiz doğrultusunda bu kritik hastalara Tablol 'de gösterildiği gibi tanı ve tedavi için sistemik bir yaklaşım önermekteyiz.

Abstract

Despite the frequency with which atherosclerosis results in occlusion of the iliac, femoral, popliteal and distal arteries of the lower extremities, acute complete occlusion of aorta is not a common occurance. This report describes 10 patients treated at Ege University, Cardiovascular Surgery Department between 1993-1998 for acute occlusion of abdominal aorta. The etiology of acute occlusion, varied clinical presentations, management and outcome were reviewed. Claducation was present all patients past history except infant patient. Varied degree of sudden onset lower extremity ischemia was most frequent presenting symptom and the trombosis was the most common cause of aortic occlusion in our series. Arterial circulation was reestablished in all patients, with the use of embolectomy in 2, embolectomy and axillobifemoral bypass in 3, transabdominal aortic trombectomy and Y graft interposition in 2, axillo-femoral bypass without embolectomy in 1, endarterectomy to visceral arteries and Y graft interposition from thoracic aorta to iliac arteries in 1. Four of 10 patients (%40) were died postoperatively due to severe acidosis and bowel ischemia. Based on our results, low ejection fractions, severe limb ischemia including parasthesia and paralysis, trombosis of visceral arteries and prior myocardial infarction were correlated with ominous prognosis. Based on our experience and review of the literature, we propose a systemic approach to the evaluation and treatment of these critically ill patients which was showed in Table 1.
