Özet

Kliniğimizde 1986-1998 yılları arasında ortalama yaşları 51.6+15.9 (21-86) olan, 9'u kadın, 19 hastaya akut aort oklüzyonu nedeni ile bilateral retrograd femoral embolektomi/trombektomi operasyonu yapıldı. Altısı akut tromboz, 13'ü embolik nedenli olan bu hastaların 7'sinde geliş şikayeti parapleji idi. Embolik gruptaki hastaların % 77'sinde atrial fibrilasyon mevcuttu. Postoperatif erken dönemde 5 olgu (% 26) kaybedildi. Mortal seyreden olgularda iskemi süresi ortalama 11.6+3.4 saat (4-24) iken, diğer olgularda bu süre 8.4±0.8 saat (4-14) idi (p=0.10). Şifa, ile taburcu edilen iki olgu daha sonra rekkürens nedeni ile postoperatif 4 ve 14. ayda tekrar ameliyat edilmişlerdir. Sonuç olarak; semptomatolojisi nedeni ile nöroloji ve nöroşirurji kliniklerinde vakit kaybedebilen bu hasta grubunun erken olarak tedavisi mortaliteyi önemli olarak azaltacak bir etkendir. Bilateral retrograd embolektomi hayat kurtarıcı olacaktır. Abdominal aortada önemli aterosklerotik lezyonu olan hastalarda, anatomik yada ekstra-anatomik bypasslar ile revaskülarizasyon tekrarlayan aort trombozunun kötü sonuçları ile karşılaşılmaması için önemlidir. Geç dönem rekkürensin önlenmesinde antikoagülan ilaçlar önerilmektedir.

Abstract

Nineteen patients, whose mean age was 51.6±15.9 (21-86), were operated in our clinic between the years 1986-1998 because of acute aortic occlusion and bilateral retrograde femoral embolectomies were performed. ) patients were females. Two groups of patients were identified; 6 cases were classified as acute thrombosis nad 13 cases were classified as embolic cases. The main complaint was paraplegia in 7 patients at admittance. Atrial fibrilation was present in 10 of 13 patients of the embolie group. Five patients (26 %) died in the early postoperative period. Mean ischemic period of lower limbs was 11.6±3.4 hours (4-24) in mortal cases, while this figure was 8.4±0.8 hours (4-14) in the remaining cases (p=0.10). Two cases were reoperated in the late postoperative period, at 4th and 14th months due to reccurent embolies. As a conclusion; these cases who are apt to waste.time before admitted to the vascular surgery clinics due to the symptoms, usually misdiagnosed as an entity that is related to neurologic pathology, should be treated promptly when diagnosed with bilateral femoral retrograde embolectomy, which really can be life saving in many cases. Anatomic or extra-anatomic bypasses may be needed in patients who have significant atherosclerotic lesions in their abdominal aorta to prevent the dramatic outcome of reccurent thrombosis. Anticoagulation can prevent late reccurences.
