Özet

Abdominal aort cerrahisi sonrası gelişen şiloperitoneum çok ender görülen bir komplikasyon olup, genellikle retroperitoneal diseksiyon sırasında majör abdominal lenfatik kanalların yaralanması sonucunda ortaya çıkmaktadır. Postoperatif drenaj veya parasentez sıvısının özellikle biyokimyasal analizi tanı koymak için yeterlidir. Tedavide, başarı şansı değişken çeşitli operatif ve nonoperatif yöntemler söz konusudur. Önerilen tedavi, yüksek protein, düşük yağ ve orta zincirli trigliserid (MCT) desteğinde total paranteral nutrisyon (TPN) dur. Yanıt alınamayan olgularda peritoneo-venöz şant veya cerrahi eksplorasyona başvurulabilir. Bu yazıda renovasküler hipertansiyon nedeniyle aynı seansta uygulanan bilateral aorto-renal safen bypass sonucu gelişen ve konservatif olarak başarı ile tedavi edilen bir Şiloperitoneum olgusu sunulmakta; bu komplikasyon etyoloji, tanı, tedavi alternatifleri ve sonuçları bakımından gözden geçirilmektedir.

Abstract

Chylous ascites is an extremely rare complication of abdominal aortic surgery. It occurs due to injury of major abdominal lymphatics during extensive retroperitoneal dissection. The diagnosis is easily confirmed by paracentesis. Many different regimens has been used to treat chyloperitoneum after aortic surgery. This include repeated therapeutic paracentesis, dietary control (with a high protein, low fat, medium chain triglyceride nMCT- diet), total paranteral nutrition (TPN), diuretics, intravenous reinfusion of ascites, peritoneo-venous shunting, and surgical closure of the lymphatic fistulas. These treatments were tried either alone or in combination with other therapeutic modalities. Conservative treatment may necessitate the need for laparatomy and ligation of leaking lymphatics or peritoneo-venous shunting. In this report, a case with chylons ascites following of aorto-renal bypass surgery is presented.
