Özet

Operasyon gerektiren karotis arter hastalarında birlikte bulunan koroner arter hastalığı veya planlanan diğer büyük operasyonların zamanlaması sıkça tartışılan bir konudur. Bu çalışmada aynı seans veya aşamalı olarak karotis endartrektomi (KAE) ameliyatı sonuçları karşılaştırıldı. Bunun için Emory Üniversitesi Hastanelerinde 1987 ile 1997 yılları arasında aynı hastane yatışında diğer operasyonlarla birlikte KAE gerçekleştirilen 165 hasta retrospektif olarak irdelendi. Bu hastalardan7110 tanesine kardiak operasyon uygulandı. Serideki 165 hastanın post operatif ilk 30 günlük felç ve ölüm oranları %14.6 olarak bulunurken, kardiak cerrahi gerçekleştirilen 110 hastada bu oranın %18.2 olduğu görüldü. Kardiak cerrahi sırasında karotis cerrahisi uygulanan hasta grubunda mortalite ve morbidite %26.2 bulunurken, ayrı seanslarda ameliyat edilen hastalarda bu oran %6.6 olarak tespit edildi. Aynı zaman biriminde sadece karotis arter hastalığı nedeniyle endartrektomi uygulanan 1125 hastanın mortalite ve morbidite %2.1 olarak bulundu ve kontrol grubu olarak kullanıldı. Karotis ve kardiak cerrahinin uygulandığı hasta grubunun preoperatif risk faktörlerinin yüksek olması ve kombine operasyonların özellikle bu grupta uygulanması, mortalite ve morbiditeyi belirgin bir şekilde arttırmaktadır. Koroner arter hastalıklarının, tedavisinin imkan olduğu sürece karotis operasyonunu takip eden günlerde yapılmasının felç ve ölüm oranını düşüreceğine inanmaktayız.

Abstract

The occurrence of significant carotid disease in patients requiring coronary revascularization or other major surgical procedures results in the dilemma of whether simultaneous or staged operations should be performed. To determine appropriate therapy, we reviewed this experience at Emory University Hospital during a 10 year period from 1987 until 1997, 165 patients underwent carotid endarterectomy during the same hospitalization or simultaneously with another surgical procedure, 110 of which were major cardiac procedures. The combined 30-day postoperative stroke and death rate was 14.6% for the 165 individuals having concomitant procedures, and of the 110 patients undergoing concomitant or same hospitalization major cardiac surgery, the combined 30 day postoperative stroke and death rate was 18,2%. When comparing morbidity and mortality rates for those having simultaneous carotid endarterectomy and coronary artery bypass to those having delayed coronary artery bypass, the latter group was found to have a 6.6% combined risk of postoperative stroke or death within 30 days while those undergoing simultaneous procedures had a 26.2% risk. In a control group of 1125 patients undergoing carotid endarterectomy alone during the same time period, the combined 30 day mortality and stroke morbidity was 2.1%. Although the patient population undergoing simultaneous carotid and coronary revascularization may have more severe disease we believe that the combination of procedures during the same operative setting results in an increased perioperative stroke and death rate. Consequently, only extremely high risk patients are selected for simultaneous procedures; otherwise, our experience suggests that delaying coronary artery bypass by several days will reduce overall postoperative mortality and stroke morbidity.
