Özet
Karotis endarterektomisi sırasında klampaj süresince serebral perfüzyonun devamını sağlamak amacı ile en sık seçilen yol şant kullanımıdır. Bir grup araştırıcı şant kullanımını rutin uygularken, bir grup şant kullanımını önermemekte ve bir diğer grup da bizim de politikamızı bu yönde şekillendirdiğimiz selektif şant kullanımını önermektedir. Çalışmamızda bu yaklaşımımızın sonuçlarını tartışmayı amaçladık. 1991-2000 tarihleri arasında 271 hastada 281 endarterektomi uygulandı. Güdük basıncı 50 mmHg'nin altında olan, hikayesinde son 3 ay içinde ipsilateral serebral enfarktüs olan ve kontrlateral karotisi tıkalı olan 106 (%38) olguda şant kullanıldı. On olguda (% 3.5) strok gelişti. Yedi olgu (% 2.49) kaybedildi. Temel kurallara bağlı kalındığında kabul edilebilir mortalite ve morbidite oranları içinde karotis cerrahisi uygulanabilir.
Abstract
The most frequent way of establishing an adequate cerebral perfusion during temporary clamping in carotid endarterectomy, is the use of indwelling shunts. Although some investigators avoid using shunts, options also include selective and routine shunting. In this report we present the results of our series, in which we have preferred selective shunting. 281 carotid endarterectomies on 271 patients were performed during 1991-2000. The determination of the need for shunting was done on the basis of the presence of an history of CVA in last 3 months, contrlateral occlusion of the internal carotid artery and a stump pressure less than 50 mmHg. Of 106 (% 38) patients selected for shunting, 10 (% 3.5) had a stroke and 7 (%2.49) cases resulted in death. Our results reveal that carotid endarterectomy can be safely done with acceptable morbidity and mortality rates if the basic rules are obided by.
