Üst ekstremite venlerinde anjioskopinin sonuçları ve infrainguinal bypass materyali olarak kullanılmalarına etkileri

Özet
Femoro - distal rekonstrüksiyonlar, Koroner revaskülerizasyonlardaki artışlar, otolog ven materyalinin distal bypass cerrahisinde kullanılabilme olanağını günden güne sınırlamaktadır. Üst ekstremite venleri bu nedenlerden dolayı gittikçe artan bir sıklıkta otolog bypass materyali olarak kullanılmaya başlanmıştır. Çalışmamızda 01/94 - 08/99 tarihleri arasında femora - distal bypass cerrahisinde (n=67) bypass materyali olarak kullanılmış olan üst ekstremite yenlerinin (136 segment) endoskopik inceleme sonuçları prospektif olarak incelenmiştir. Bypass materyali olarak çapı 2.5 mm'den büyük olan venler kullanılmıştır, incelenen 136 ven segmentinde 43 patolojik endoluminal bulguya rastlanmıştır. Web (n=28), trombus (n=6), ven sklerozu/stenozu (n=9). Patolojik bulgulara önkolda V. Cefalica'da % 41.4, V. Basilica'da % 33.3, V. mediana cubiti'de % 46.6, üst kolda V. Cefalica'da % 28.8, V. Basilica'da ise % 18.8 oranlarında rastlanılmıştır. Anjioskopi eşliğinde yürütülen manipülasyonlar [eksizyon (12), patchplasti (4), trombektomi (5), web rezeksiyonu (21)] sayesinde lezyonların büyük bir çoğunluğu düzeltilebilmiştir. Bypass rekonstrüksiyonlarında, veno-venostomi den mümkün olduğunca kaçınılacak veya en az düzeyde tutulacak şekilde ve ortalama olarak 2.3 segment kullanılmıştır. Transplant materyalleri çoğunlukla reverse edilerek implante edilmişlerdir. Rekonstrüksiyonların % 86.8'i crural veya pedal düzeylerde idi. Üst ekstremite venlerinin rutin olarak femoro-crural rekonstrüksiyonlarda kullanılması mümkündür. Endoskopik incelemeler, kullanılacak ven segmentinin kalitesi hakkında çok değerli bilgiler sunmaktadır ve bu bilgiler eşliğinde yapılan rekonstrüksiyonların sonuçları da endoskopinin önemini ortaya çıkaran niteliktedir.
Abstract
The possibility of the usage of the autologue vein material in distal bypass surgery is decreasing more and more because of Femoro-distal reconstructions and the increases in Coronary revascularisations. Arm veins are more often being used as autologue bypass material due to these causes. In this study, the results of endoscopic examinations of arm veins (136 segments) which have been used as bypass material in Femoro-distal bypass surgery (n=67) between 01/94 - 08/99 have been prospectively examined. Veins with diameters bigger than 2.5 mm have been used as bypass material. Among 136 vein segments examined, 43 patological endoluminal abnormalities have been diagnosed. Web (n=28), thrombus (n=6), vein sclerosis/stenosis (n=9). The patological endoluminal abnormalities have been diagnosed in the lower arm are as 41.4 % in the Cephalic vein, 33.3 % in the Basilic vein, and 46.6 % in the Cubital vein; in the upper arm as 28.8 % in Cephalic vein, 18.8 % in the Basilic vein. It has been possible to repair a big number of these endoluminal abnormalities with the manipulations accompanied by angioscopy [excision (12), patchplasty (4), thrombectomy (5), web resection (21)]. In these bypass reconstructions, veno-venostomy has been avoided as much as possible. The median of the segments used per reconstruction is 2.3. The transplant materials have mostly been reversedly implanted. 86.8 % of the reconstructions were crural or pedal reconstructions. It is possible for the arm veins to be used in Femoro-crural reconstructions as a routin autologue bypass material. Angioscopy presents valuable information about the quality of the vein segment to be used; and the results of the reconstructions made by relying on this information prove the importance of endoscopy.
