Özet

Bu çalışmada akut periferik arter tıkanmalarının cerrahi tedavisi sonrasında tekrarlayıcı embolilerin önlenmesinde klasik ve düşük molekül ağırlıklı heparinin (DMAH) etkilerinin karşılaştırılması amaçlanmıştır. Bu amaçla kliniğimizde ocak 1995 - mart 1999 tarihleri arasında akut periferik arteryel emboli tanısıyla ameliyat edilen 255 olgu incelenmiştir. 167 hastaya girişim sonrası unfraksiyone heparin (UF), 88 hastaya ise düşük molekül ağırlıklı heparin (DMAH) uygulanmış ve bu gruplar rekürren emboli açısından takip edilmiştir. Unfraksiyone heparin (UF) grubunda 45 hastaya (%27) sekonder girişim yapılmıştır.11 hastada (%6.5) rekürren emboli saptanmış, 7 hastada (%4) kanama komplikasyonu gelişmiştir.Bu grupta amputasyon oranı%14, mortalite %11 bulunmuştur. DMAH grubunda 21 hastaya(%24) sekonder girişim uygulanmış, 5 hastada (%5.6) rekürren emboli saptanmıştır. Bu grupta 3 hastada (%3.4) kanama komplikasyonu gelişmiştir.Amputasyon oranı %12.5, mortalite %14.7 bulunmuştur. Sonuç olarak akut periferik damar iskemisi nedeni ile cerrahi girişim yapılan hastalarda rekürren embolinin önlenmesinde unfraksiyone heparin ile düşük molekül ağırlıklı heparin arasında belirgin bir fark olmamasına rağmen, düşük molekül ağırlıklı heparinlerin kullanım ve takip kolaylığı açısından daha avantajlı olduğu kanısına varılmıştır.

Abstract

In this study we compared the effects of unfractionated heparin(UH) with low molecular weight heparin(LMWH) in the prophylaxis of recurrent peripheral arterial thromboembolisms after surgical intervention. We analysed 255 cases of acute peripheral arterial embolism who were treated surgically, retrospectively. After the operation 167 patients were treated with UH, whereas 88 had LMWH injections and , both groups were observed for recurrent episodes. In the UH group 45 patients (27%) were operated secodarily.11 patients (6.5%) had a recurrent emboli, and 7 (4%) had bleeding. In this group the amputation rate is14%,and the mortality rate is 11%. In the LMWH group,21 patients (24%) had a second operation, and recurrent emboli was found in 5 (5.6%) of them. 3 Patients (3.4%) had a bleeding event. The amputation rate is 12.5%, and the mortality rate is14.7%. As a result, although there is not a sensible difference between unfractionated heparin arid low molecular weight heparin as a prophylactic agent, in the prevention of recurrent embolic attacks after surgery for acute peripheral arterial thromboembolism, low molecular weight heparins are proved to be more advantageous being easer to use and follow up.
