Ratlarda venöz trombozis süresi ile yeni vasküler gelişmeler ve trombolizisin ilişkisi

Özet
Bu çalışmanın amacı, ratlarda oluşturulan derin ven trombozu sonrası meydana gelen neovaskülarizasyon ile trombozis süresi ve trombolizis arasındaki ilişkileri araştırmaktır. 20 Sprague-Dawley ratın bilateral femoral venleri disseke edildikten sonra, kontrol grubunu oluşturmak amacıyla ratların sağ femoral venlerine oklüzyon uygulanmadı. Akut derin ven trombozu oluşturmak amacıyla sol femoral ven 10 ratta proksimalden 4/0 ipek ile bağlanırken (Grup 1) diğer 10 ratta ise kliplendi. Klip, 5 ratta 24 saat sonra (Grup 2A), diğer 5 ratta ise 72 saat sonra kaldırıldı (Grup 2B). Kronik venöz obstrüksiyon oluşturulan ratlarda (Grup 1 ve Grup 2B) erken dönemde trombüsün yayılması ve 7-15. günlerde pik yapan hücresel proliferasyon dikkati çekmekteydi. 15-18 gün içerisinde trombolizis sonucu gelişen rekanalizasyon sonrasında neointimal proliferasyonun gerilediği, ancak medial hücresel büyümenin devam ettiği gözlendi. Geçici venöz obstrüksiyon oluşturulan ratlarda (Grup 2A) ise, erken dönemde trombus yayılması gözlenmez iken, 6-8 gün içinde trombolizis ve rekanalizasyon oluştuğu, 15 gün içinde de tüm proçesin normale döndüğü gözlendi. Yeni vasküler oluşuma ise rastlanmadı. Ratlarda kronik venöz obstrüksiyona bağlı gelişen trombozis, obstrüksiyon süresine paralel olarak trombozun erkenden yayılmasına, trombolizisin ve dolayısı ile rekanalizasyonun gecikmesine ve yeni vasküler yapıların gelişmesine yol açmaktadır.
Abstract
Acute deep venous thrombosis was developed in rats to assess the relations among the duration of deep vein throm-bosis, neovascular growths and thrombolysis. Both femoral veins of 20 male Sprague-Dawley rats were dissected. Right femoral veins served as control group. The left femoral veins were proximally obstructed to cause thrombosis. Obstruction was achieved either by means of pro-ximal suture ligation in the 10 rats (Group 1) or by means of clip ligation. Clip was removed in 5 rats after 24 hours (Group 2A) and in the remaining 5 rats after 72 hours (Group 2B). Group 1 and Group 2B veins had marked vein wall thickening consisting of smooth muscle type, neointimal and cel-lular proliferation peaking between 7 and 15 days. Within 15-18 days, neovascular growths were observed, and re-sumption of normal venous flow was seen to bypass the obstructed segment. Subsequently, neointimal proliferation regressed, but medial cellular growth persisted. In contrast, group 2A veins had no neovascular changes and early thrombosis extension was seen (within 8 days). Within 15 days, Group 2A veins regained normal appearance and resumed normal flow. Thrombosis with chronic venous obstruction results in early extension of thrombus, delayed re-canalization, development of neovascular channels and permanent proliferative changes
