Özet

Ekstrakranial karotis arterlerinin atherosklerotik tıkayıcı hastalığında cerrahi uygulamanın temel amacı inmelerin önlenmesidir. Bu çalışmada cerrahi endikasyonlar ve eşlik eden hastalıklar göz önünde bulundurularak endarterektomi sonuçları incelenmiş ve literatür gözden geçirilmiştir. 1988-1997 yılları arasında 22 hastaya 24 endarterektomi uygulanmıştır. Geçici iskemik atak (GİA) 15 hastayla (%68) ilk sırayı alarak en önemli ameliyat endikasyonunu oluşturmaktadır. 4 hastada (%18) eşlik eden koroner arter hastalığı nedeniyle 3'üne öncesinde 1 'ine aynı seansta koroner baypas uygulanmıştır. Yine 2 hastada (%9) eşlik eden aorta-iliak tıkayıcı hastalık nedeniyle 1 'ine aynı seansta diğerine sonrasında aorta-bifemoral baypas yapılmıştır. Peroperatif mortalite bir hastadaki myokart enfarktüsü nedeniyle %4 ve postoperatif geçici nörolojik morbidite %9 olarak saptanmıştır. GİA geçirmiş hastaların inme riskini azaltmak için en uygun hasta grubunu oluşturduğu açıktır. Fakat eşlik eden ciddi koroner arter hastalığının kabul edilebilir olan mortaliteyi yükselttiği unutulmamalıdır.

Abstract

The primary reason of surgery for atherosclerotic occlusive disease of extracranial carotid arteries is prevention of stroke. In this study, endarterectomy results and literature were reviewed with implication of surgical indications and accompanying diseases. 24 endarterectomies were applied to 22 patients between 1988-1997. Transient ischeamic attacks (TIA) taking first rank with 15 patients (68%) were the most important' indication for the surgery. Because of the accompanying coro-nary artery disease, 3 patients previously and 1 patient at the same session, totally 4 patients (18%) were undergo-ne to coronary artery bypass surgery. And also, 2 patients (9%) because of an accompanying aortoiliac occlusive di-sease were undergone aorto-bifemoral bypass operation at the same session in one and afterwards in another. Temporary neurologic morbidity of 9% and perioperative mortality of 4% due to a myocardial infarction in one patient was found. It is clear that patients with the TIA are the most suitable group to decrease the risk of stroke. But, it was not forgotten that accompanying serious coronary artery disease increased the acceptable mortality for this disease
