Özet
Amaç: Resusitasyon,hasta transportu ve vasküler cerrahi teknikteki gelişmelere rağmen popliteal vasküler travmalı (PVT) hastalarda amputasyon oranı hala yüksek seyretmektedir.Bu konudaki deneyim ve görüşlerimiz çalışmamızın amacını oluşturmaktadır. Materyal ve Metod: Kliniğimizde Ocak 1993-Aralık 1997 yılları arasındaki 5 yıllık periyodda 21 popliteal vasküler travmalı olgu retrospektif olarak incelendi. Olguların 20 si erkek (%95), 1'i kadın (%5) ve yaş ortalamaları 31.2 (7-65) idi. Olguların 13'ü penetran (%62), 8'i künt (%38) travma ile başvurdu.Tanısı konulan olgulara gerekli rekonstrüktif yöntemler (reversed safen ven by-pass, primer anastomoz lateral sütür, fistül tamiri, ligasyon) uygulamıştır. Sonuç: Mortalitemiz yoktur. Dört olguda toplam 6 kez tromboembolektomi yapılmış olup, 3 olguda erken 5 olguda geç dönemde olmak üzere toplam 8 olguda (%38) amputasyon yapılmıştır. Tartışma: PVT'lı olgular tanınması zor yaralanmalardır. Bu olgularda rekonstriksiyona rağmen amputasyon oranının yüksek seyretmesinin başlıca nedenleri arasında tanıdaki ve tedavideki gecikme (özellikle ortopedi kliniklerinde damar yaralanması olduğu bilinmeden olguların uzun süre bekletilmesi) ve önceden yapılan başarısız girişimlerdir.Sonucun başarılı olmasında erken tanı ve cerrahi tekniğin önemi aşikardır. Bu yüzden bu olgular şüphelenildiği zaman vakit kaybedilmeden damar cerrahisi merkezine gönderilmelidirler.
Abstract
Aim: In spite of new developments in resuscitation, patient transportation and vascular surgery techniques, amputation ratio is still high in patients with popliteal vascular traumas (PVT). The aim of this study is to report our experience on this subject Material and Methods: We retrospectively examined 21 patients with PVT who were referred to our department between January 1993-December 1997. There were 20 males and 1 female aged between 7-65 years and median age was 31,2. Eight patients had blunt trauma, the remaining 13 patients had penetration injury. All the patients were tre-ated with reconstructive surgical methods (reversed saphenous vein by-pass, primary anastomosis, fistula repair and ligation) Results: In four patients, six thromboembolectomy were performed totally. Eight amputations were performed out of 21 patients. Three of them were at early period whereas five amputations were performed at later periods. No mortality occured. Conclusion: The reasons for the high amputation rate in PVT patients treated with reconstructive methods are delays in diagnosis and defective treatment (especially delays in surgical intervention to the patients primarily observed by orthopaedic surgeons, if vascular injury is not suspected). Early diagnosis and proper surgical techniques are important for better outcome. The patients with suspected vascular injury must be transferred to vascular surgery centers as soon as possible.
