Özet

Vertebral arter cerrahisi Türkiye'de pek yapılamayan cerrahi girişimlerdir ancak vertebro-baziler iskemi bulguları olan ve semptomların kaynağının vertebro-baziler 'dolaşım sisteminden kaynaklandığı ispat edilen olgularda tatminkar sonuçlar vermektedir. Bu bildirimizde iki olgu zemininde semptomatik vertebral arter Stenozlannda cerrahi girişimi tartışmayı amaçladık. 61 yaşında kadın ve 77 yaşında erkek iki olgu denge kaybı ve düşme atakları ile başvurdu. Anjiyografide iki olguda da vertebral arter çıkışında kritik darlık tespit edildi. Transluminal anjiyoplastinin başarısız olması üzerine vertebro-karotid.reimplantasyon uygulandı.Ameliyat sonrası iki olgu 2. ve 3.günlerinde asetil salisilik asit ile problemsiz taburcu edildi. Şikayetleri gerileyen olgular takiplerinin 4.yılında ve 15.ayında sorunsuzlar. Sonuç olarak vertebrobaziler yetersizlik tablosuna yol açan, diğer sistemik nedenlerin ayırıcı tanıda dışlandığı vertebral arter darlıklarında yapılan girişimler tatminkar sonuçlar vermekte ve hastanın hayat kalitesini yükseltmektedir.

Abstract

Vertebral artery surgery is seldom performed in Turkey. However these procedures reveal satisfactory results in patients with signs and symptoms proven to be â result insufficiency of vertebro-basilary circulation. This report aims to discuss the effectiveness of surgery in symptomatic vertebral artery stenosis on the basis of two cases. A61 years old female and a 77 years old male were admitted to the hospital because of repeated attacks of balance loss, and fall. Investigations revealed critiçal,.stenoşis. of. the vertebral artery at the orifice, iabqth of.the cases and transiuminal balloon angioplasty was planned. Vertebra-carotid reimplantation was performed in both cases after the angioplasty interventions both failed. Both cases were discharged from the hospital on the 2nd and 3rd days of the operation on acetyl-salicylic-acid. Follow-ups at 4th year and 15th month revealed improved outcomes without any problem. In conclusion vertebral artery surgery has good results in patients with symptomatic vertebro-basilary circulation disorders and improves life quality.
