Akut tip B aort diseksiyonunda yaklaşım ve cerrahi tedavi sonuçları

Özet
Akut tip B diseksiyonlarda mortalite ve morbidite oranlan tip A diseksiyonlara göre halen yüksektir. Özellikle komplike olmayan akut tip B diseksiyonlarda yapılan cerrahi tedavinin medikal tedaviye üstün olmaması cerrahi olgu sayısını sınırlamakta ve yeterli tecrübe kazanılmasını engellemektedir. Kliniğimizde Ocak 1996- Kasım 2000 tarihleri arasında akut tip B disekan aort anevrizması nedeni ile başvuran 15 olgudan 10'una cerrahi müdahale yapıldı. 5 olgumuzda medikal tedaviye dirençli ağrının devam etmesi, 2.olgumuzda büyük anevrizma (1'inde 6 cm, diğerinde 5 cm ve Marfan sendromu bulguları mevcuttu), 2 olgumuzda periferik dolaşım ve visserai organ malperfüzypnu, 1 olgumuzda antihipertansif tedaviye rağmen düşmeyen hipertansiyon nedeni ile operasyon kararı verildi. Hiçbir problemi olmayan 5 hastamız medikal tedavi ile takip altına alındı. Operatif mortalite oranımız, 2 olgu ile % 20 oldu. Bunlardan 1 olgumuz postoperatif 30. gün yeni bir hemoraji, diğer olgumuz da postoperatif 25. günde multiorgan yetersizliğinden kaybedilmiştir. Bir olgumuzda da geçici monoparezi gözlendi. Torasik aort cerrahisinde en önemli konu başta spinal kord olmak jzere vital organların iskemik reperfüzyon hasarıdır. Bu hasarı engellemek ve 45 dakikayı geçen kross klemp sürelerinde distal perfüzyonun sağlanması önerilmektedir. Özellikle arkus aort yırtığından kaynaklanan distal disseksiyon, proksimal klemp hasarı, ileri derece aort kalsifikasyonu gibi klinik durumlarda total sirkülatuvar arrest'gerekebilir. En önemli avantajı gerektiğinde total sirkulatuvar arreste girilebilme imkanı olan femoro-femoral parsiyel kardiyo-pulmoner by-pass'ın akut tip B diseksiyon cerrahisinde başarı ile uygulanabileceğini düşünüyoruz.
Abstract
The morbidity and mortality rates in acute type B dissections remain still higher than in acute type A dissections. The controversies that still continue concerning the optimal mode of treatment for uncomplicated type B dissections limi-ted the number of cases managed surgically. At our institution, between January 1996-November 2000, we performed surgical intervention to 10 of 15 patients who had the diagnosis of an acute type B dissecting aortic aneurysm. Primary indications for surgery were the presence :Of.continued pain despite the medical treatment in 5 patients, large aneursms in 2 patients ( in one patient the diameter of the aorta was 6 cm, in the other patient with the diagnosis of Marfan 's syndrome the diameter was 5 cm), périphérie and visceral organ malperfusion in 2 patients and uncontrollable hypertension in the last patient. Five patients without any problem were managed medically. Mortality rate was % 20. The first patient was lost at the end of postoperative day 30 because of a new bleeding. The other patient was lost because of multiorgan failure at the end of postoperative day 25th. In one of the remaning patient, we observed a transient monoparesis . The most important concept in thoracic aortic surgery is the reperfusion injury of the vital organs including the spinal cord. In order to prevent the post-operative catastrophic complications related to reperfusion injury, many authors proposed the use of a distal perfusion technique especially in operations with cross-clamp times exceeding 45 minutes. In cases with severely calcified aortic wall and also in patients with a tear in aortic arch, the surgeon may need the establishment of total circulatory arrest. Among several methods proposed for distal perfusion, partial femoro-femoral cardiopulmonary bypass has the advantage of immediate establishment of total circulatory arrest. So, we believe that the partial femoro-femoral cardiopulmonary by-pass is the succesfull method that may be used in the.surgical management of type B dissections.
