Özet
Amaç: Femoral arter anevrizmaları nispeten nadir görülen patolojilerdir. Prognozumm iyi olduğu bilinmekle beraber tedavileri ve postoperatif seyirleri hakkında çok fazla çalışma yoktur. Biz bu yazımızda, kliniğimizde Ocak 2000 - Eylül 2006 yılları arasında nadir görülen bir patoloji olan gerçek FAA tanısı konan ve ameliyat edilen 7 olgumuzu bildirmekteyiz. Yöntem: Cutler ve Darling'in yaptıkları sınıflamaya göre hastalardan üçünde sadece Tip I femoral arter anevrizması, birisinde sadece Tip II femoral arter anevrizması, birisinde bilateral yüzeyel femoral arter anevrizması, birisinde sağ tarafta Tip I ve sol tarafta Tip II femoral arter anevrizması, birisinde ise solda Tip I femoral arter anevrizması ve sağda izole derin femoral arter anevrizması vardı. Bir hastada distal mikroembolizasyon varken üç hastada hiç semptom yoktu. Bulgular: Tüm hastalar genel anestezi altında ameliyat edildiler. Tedavi için sentetik greft interpozisyonu uygulandı. Derin femoral arter anevrizması olan bir hastada ligasyon uygulandı. Bilateral yüzeyel femoral arter anevrizması olan bir hastada ise tek taraflı müdahale uygulandı ve diğer taraftaki anevrizması çok büyük olmadığından tıbbi takibe alındı. Hiçbir hastada mortalite ve morbidite gelişmedi. Ekstremite kaybı olmadı. Sonuç: Femoral arterin gerçek anevrizması nadiren komplikasyon geliştirmesine rağmen kolayca tedavi edilebilen bir patoloji olduğundan ve anevrizma boyutu büyüdüğünde rüptüre ve trombüs gelişimine bağlı olarak komplikasyon oranları artabileceğinden cerrahi tedavi ertelenmemelidir.
Abstract
Background: True aneurysm of the femoral artery is a rare pathology. However some authors reported that the outcome of the patients with femoral aneurysm are relatively benign, there are limited study about treatment strategies and postoperative course. In this report, we describe seven case of true atherosclerotic aneurysm of femoral artery and evaluate surgical results. Methods: Data were collected on a total of 7 consecutive patients undergoing surgical repair of femoral true aneurysm between 1 January 2000 and 1 September 2006 in our clinic. According to Cutler and Darling Classification there was type I femoral artery aneurysm in tree patients, type II in one patient, bilateral superficial femoral artery aneurysm in one, type I on right extremity and type II on left extremity in one patient. The last patient had left type I femoral artery aneurysm and right isolated deep femoral artery aneurysm. One patient had symptoms related to microembolization. There was no symptom in tree patients. Result: All patients were operated under general anesthesia and endotracheal intubation. We performed deep femoral artery ligation in one patient and synthetic graft interposition in others. Surgical intervention was performed on only one extremity inpatient with bilateral superficial femoral arteiy aneurysm. The other anewysm was too small; surgical intervention was not perfoimed. There was no mortality, morbidity, and amputation. Conclusion: Although complications related to true femoral artery anewysms are rare, but surgical intei-vention is simple and safe. Considering the risk of limb-threatening complications like as rupture or distal embolization early diagnosis and surgical intervention are recommended.
