Özet
Venöz Tromboembolizm (VTE) klinik bulgu vermeyen bald›r ven trombozundan ekstremite kayb›na kadar gidebilen ve akci¤er embolisi gibi ölümcül komplikasyonlara yol açabilen bir hastal›kt›r. Pulmoner emboli hastanede yatan hastalarda ortaya ç›kabilen ve hayat› tehdit eden bir komplikasyondur. Günümüzde VTE’nin önlenmesi ve tedavisinde önemli aßamalar kaydedilmesine ra¤men, pulmoner embolizm hala erken hastane ölümleri aras›nda en s›k rastlanan ölüm sebebidir. Kliniklerde standart proflaksi protokolünün olußturulmamas›, gerek edinsel gerekse konjenital nedenlerden dolay› risk alt›ndaki hastalar›n tam olarak belirlenememesi VTE’nin hala yüksek morbidite ve mortalite ile seyretmesine neden olmaktad›r. VTE’nin etyolojisi çok çeßitli nedenlerden olußur. VTE ile genetik determinantlar›n ilißkisi günümüzde daha iyi bilinmektedir. VTE ile ilißkili olan genetik mutasyonlar ve polimorfizmlerin ço¤u koagülan ve antikoagülan proteinlerde bulunmaktad›r. Renin- Anjiotensin sistemindeki (RAS) genetik de¤ißikliklerin günümüzde VTE olußumunda rol oynayabilece¤i ileri sürülmektedir. Bu derleme RAS’de en çok araßt›r›lm›ß olan anjiotensin converting enzim (ACE) insersiyon/delesyon (I/D) polimorfizminin ve plazma ACE düzeylerinin venöz tromboembolizm ile olan ilißkisini anlatmaktad›r.
Abstract
Venous thromboembolism is a disease which varies from asymptomatic calf vein thrombosis to mortal complications like extremity loose and pulmonary embolism (PE). Despite of progresses on prevention and therapy of VTE, PE is the most cause of early hospital deaths. Ethiology of VTE varies. The relationship between VTE and genetic determinants is understood in recent studies. Most of the genetic mutations and polimorphisms related with VTE are exist on coagulant and anticoagulant proteins. In recent studies it is argued that the genetic determinants of Renin Angiotensin System (RAS) have role in VTE formation. In this review the relationship of VTE with the most investigated Angiotensin-Converting Enzym (ACE) Insertion/Deletion (I/D) polimorphism and plasma ACE levels is described.
