Özet

Leriche sendromu aorta bifürkasyonunun total oklüzyonu olarak tanımlanır. Alt ekstremitede klaudikasyon, erektil disfonksiyon ve femoral pulsasyonların kaybı ile ortaya çıkan klinik semptomları içeren bir terimdir. Bu olgu bildirisinde Leriche sendromu tanısı alan 67 yaşındaki erkek hastanın 3BMRA (3 boyutlu manyetik rezonans anjiyografi) incelemesi ile alt ekstremite damarlarının akımlarının ve lümen yapısının rutin DSA incelemesine göre daha iyi değerlendirilebileceği sunuldu. DSA incelemesinde alt ekstremite distal arter akımları görüntülenemeyen hastada 3BMRA incelemesi ile alt ekstremitede regüler distal akım belirlendi. Hastaya Dacron pantolon greft ile aortobifemoral baypas operasyonu uygulandı. Postoperatif süreçte alt ekstremite distal nabızları alınan hasta, postoperatif 6. Günde taburcu edildi.

Abstract

The term Leriche syndrome is commonly used to describe a variety of clinical symptoms attributed to the obstruction of the infrarenal aorta. Common clinical symptoms include thigh, hip, and buttock claudication as well as erectile impotence, usually in association with diminished femoral pulses. Our case determined the diagnostic utility of contrast-enhanced 3D MRA in patients with Leriche syndrome. The ability of 3D MRA to depict the level of aortic occlusion, the most proximal graftable arterial segments, and the type and extent of collateral pathways was shown in a 67-year-old male patient whose distal flows weren’t shown by DSA. The patient underwent aortabifemoral bypass operation with bifurcated Dacron graft and was discharged on the 6th postoperative day with palpable distal pulses.
