Özet

ÖZET Amaç: e-PTFE (expanded politetrafluroetilene) vasküler greft ile arteriovenöz fistül yapılması primer fistülün yetersiz olduğu hastalarda hemodiyaliz için girişe izin verir.Biz bu prostetik greftle ilgili klinik deneyimlerimizi ortaya koyduk. Gereç ve Yöntemler: Eylül 2003 ve Kasım 2007 arasında kronik böbrek yetmezlikli ve hemodiyaliz gereken 35 hastada e-PTFE greft ile arteriovenöz fistül yaptık. Hastaların ortalama yaşı 55.09±12.2 (23 ile 74 yaş arası), 16 (%45.7)'sı kadın, 19 (%54.3)'u erkek idi. Tüm hastalarda üst ekstremitede önceden direkt AVF yapılmıştı ve bu AVF'ler fonksiyonel değildi. Ameliyat öncesi tüm hastalarda üst ve alt ekstremite arteriyel ve venöz sistem doppler ultrasonografik tetkiki yapıldı. Hastalarda arterial anastomoz bölgesinde 4 mm, venöz anastomoz bölgesinde 7 mm çapı olan e-PTFE (Gore-Intering vasküler greft) greft kullanıldı. Bulgular 24 (%68.6) hastada brakial arter ile aksiller ven arasında, 6 (%17.1) hastada radial arter ile antekübital ven arasında, 5 (%14.3) hastada common femoral veya proksimal süperfisial femoral arter ile common femoral ven veya proksimal süperfisial ven arasında e-PTFE greft ile AVF oluşturuldu. Tüm hastalarda primer greft açıklık süresi 6.ayda %81.9,12.ayda %66.6, 24.ayda %26.6 idi. Tüm hastalarda sekonder greft açıklık oranı ö.ayda %78.2, 12.ayda %33.3, 24.ayda %17.6 idi. Perioperatif morbidite ve mortalite görülmedi. Ortalama hasta takip süresi 27.22±12.04 ay (2 ile 48 ay arası) idi. Sonuç: e-PTFE greft hemodiyaliz için tatminkar açıklık oranlan sağlar. Ameliyat öncesi doppler ultrasonogram ve venografi ile AVF için olası konfigürasyonları ortaya koymak önemli bir kriterdir.Tromboz en sık komplikasyondur.

Abstract

ABSTRACT Background: A arteriovenous fistula performing with e-PTFE (expanded polytetrafluoroethylene) vascular graft allows vascular access for hemodialysis where a primary fistula is not possible . We aimed to evaluate our clinical experience related to this prosthetic graft. Material and Methods: We performed arteriovenous fistula using e (expanded)-PTFE graft in the 35 patients with end-stage renal failure requiring hemodialysis between September 2003 and November 2007. The mean age of patients were 55.09±12.2 (range,23 to 74). There were 16 women and 19 men. Beforehand in all patients were performed direct arteriovenous fistula in the upper extremity and these arteriovenous fistulas wasn't functional. All patients were performed doppler ultrasound examination of the upper and lower extremity arteries and veins. The patients were used e-PTFE (Gore Intering vascular graft) graft 7 mm in diameter tapered to 4mm at the arterial site. Results: Between brachial artery and axillary vein in 24 patients (68.6%), between radial artery and antecubital vein in 6 patients (17.1%), between femoral artery and femoral vein in 5 patients (14.3%) were performed arteriovenous fistula. In all patients primary patency rate were 81.9%, 66.6%,and 26.6% at 6, 12, and 24 months. Secondary patency rates at 6, 12, and 24 months were 78.2%, 33.3%, and 17.6%. There were no perioperative morbidity and mortality. Mean follow-up was 27.22±12.04 months (range, 2 to 48 months). Conclusion: e-PTFE grafts provide satisfactory patency rate. Preoperative doppler ultrasonography and venography examination are important for showing of probable graft configurations. Thrombosis is the most complication.
