Çocuklarda Subkutan Port Kateter İmplantasyonuna Bağlı Komplikasyonlar ve Tedavi Stratejileri

Özet

ÖZET Amaç: Çocuk hastalarda implante edilen subkutan port kateterlerin yerleştirilmesi ve kulla-- nımı sırasında karşılaşılan komplikasyonlar ve bunlarla mücadele konusundaki deneyimlerimizi li-- teratür ışığında incelemeyi amaçladık. Gereç ve Yöntemler: Kliniğimizde Kasım 2010 Eylül 2016 tarihleri arasında 267 hastaya yerleştirilen 295 adet implante edilebilir subkutan port kateter olgu-- sunun hastane kayıtları geriye dönük olarak tarandı. Hastaların demografik verileri, port boyutları, girişimin lokalizasyonu, kullanılan teknik, girişim ile ilgili perioperatif ve uzun dönemde karşılaşı-- lan komplikasyonlar ve portun çıkartılma nedenleri incelendi. Bulgular: Hastaların 122'si kız ve 17311 erkekti, yaş ortalaları 6,1 4,3 yıl (1 ay--17 yıl) idi. Toplam komplikasyon oranı %12,2 (n=36), en sık gö-- rülen komplikasyon ise %5,4 (n=16) ile port enfeksiyonu idi. Bunların %7,8'i (n=23) internal juguler ven, %4,4'fi (n=13) subklaviyan ven girişimi sonrasında meydana geldi. Perioperatif komplikasyonlar ve sıklıkları; pnömotoraks %1 (n=3), hemotoraks 0/00,7 (n=2), malpozisyon %0,7 (n=2), arter yaralan-- ması %0,3 (n=1) ve aritmi %0,3 (n=1) olarak; uzun dönem komplikasyonlar ise port enfeksiyonu %5,4 (n=16), port trombozu %35 (n=10) ve port kırılması %0,3 (n=1) olarak bulundu. Olguların 0/o9,5'inde (n=28) komplikasyon nedeniyle port kateter çıkartıldı. Tüm komplikasyonlar açısından karşılaştırıldı-- ğında juguler ve subklaviyan vene implantasyon arasında istatistiksel olarak anlamlı fark bulunmadı. Sonuç: Uzun süreli tedavi alan çocuk hastalarda subkutan port kateter yerleştirilmesi tercih edilen, gü-- venli bir yöntemdir. Port kateterlerin kullanımının artmasıyla birlikte bunlarla ilişkili komplikasyon-- larda da artış olduğu görülmektedir. Hekim, hemşire ve hasta ailesinin bilgilerinin arttırılması ve güncellenmesi ile komplikasyon oranlarının azalacağını düşünmekteyiz.

Abstract

AABBSSTTRRAACCTT OObbjjeeccttiivvee:: The aim of this study was to analyze the complications encountered during placement and use of subcutaneously implanted port catheters, and to review our experience on management of those complications in the light of the literature. MMaatteerriiaall aanndd MMeetthhooddss:: Medical records of 295 implantable subcutaneous port catheters placed in 267 patients between November 2010 and September 2016 in our clinic were retrospectively reviewed. Demographics, port dimen- sion, localization of the intervention, the technique used, complications encountered periopera- tively and in the long-term, and the causes of port removal were analyzed. RReessuullttss:: The numbers of females and males were 122 and 173, respectively, and the mean age of the patients was 6.1 ± 4.3 years (1 month-17 years). Total complication rate was 12.2% (n=36), and the most common com- plication was port infection (5.4%, n=16). Among the port infections, 7.8% (n=23) and 4.4% (n=13) developed at the intervention site, at internal jugular vein and subclavian vein, respectively. Peri- operative complications and their rates were pneumothorax 1% (n=3), hemothorax 0.7% (n=2), malposition 0.7% (n=2), arterial injury 0.3% (n=1) and arrhythmia 0.3% (n=1). Long-term compli- cations and their rates were port infection 5.4% (n=16), port thrombosis 3.5% (n=10), and broken port 0.3% (n=1). Port catheter was removed in 9.5% (n=28) of the cases due to complications. No statistically significant differences were found in overall complications, between the localization of the intervention, namely jugular and subclavian veins. CCoonncclluussiioonn:: Implantation of subcutaneous port catheters in pediatric patients who are pllaned to have any long-term treatment is a safe and preferred method. Increasing use of port catheters has resulted in an increased rate of associated complications. Notification and update of the knowledge of the physicians, nurses and the families of the patients may decrease the complication rate
