Use of Transthoracic Echocardiography for Central Venous Catheter Placement in a Pregnant Woman

Özet
Santral venöz kateterlerin uygun yerleşimde olduğu floroskopi, anjiyografi ve akciğer grafisi gibi yöntemler ile doğrulanır. Hastanın hamile olduğu durumlarda bu yöntemlerin kullanılması sakıncalıdır. Bu yöntemlerin yerine transtorasik ekokardiyografi rahatlıkla kullanılabilir. Transtorasik ekokardiyografi yardımı ile kılavuz telin uygun yerleşimi, cilt-kava atriyal bileşke mesafesinin ölçülmesi ve bu mesafe doğrultusunda kateterin yerleştirilerek yerinin doğrulanması ile gelişebilecek komplikasyonların en aza indirilebilineceği düşüncesindeyiz. Bu yazıda ilk gebeliğinde anti-D immünoglobülin yapılmamış anneye plazmaferez yapılması amacıyla santral venöz hemodiyaliz kateterinin transtorasik ekokardiyografi yardımıyla yerleştirildiği vaka sunulmuştur.
Abstract
Proper placement of central venous catheters is confirmed by methods such as fluoroscopy, angiography and chest X-ray. In case of pregnant patients, use of these methods has drawbacks. Transthoracic echocardiography can easily be used instead of these methods. With the help of transthoracic echocardiography, proper placement of the guide wire, skin-cava-atrial junction distance measurement and placing the catheter in the direction with the verification of the location can be achieved, and possible complications can be minimized. In this case report, we present a mother, in whom anti-D immunoglobulin was not applied in the first pregnancy, and a central venous hemodialysis catheter inserted with the guidance of transthoracic echocardiography for the purpose of plasmapheresis.
