Özet

Amaç: Bu çalışmada heparin bağlı greftlerin alt ekstremite baypasları için otolog venler kadar iyi bir alternatif olup olmadıkları araştırıldı.Hastalar ve Yöntemler: Ocak 2002 - Mart 2016 tarihleri arasında diz üst femoropopliteal baypas yapılan toplam 249 hasta (169 erkek, 80 kadın; ort. yaş 66.2 yıl; dağılım, 55-73 yıl) retrospektif olarak incelendi. Hastaların 41'inde heparin bağlı politetrafloroetilen (PTFE) greft, 136'sında standart PTFE ve 72'sinde otolog ven greft kullanıldı. Birinci, ikinci ve üçüncü yıl açıklık oranları değerlendirildi.Bulgular: Birinci yıl açık kalma oranları en yüksek otolog ven greftlerde iken (%94.3), bunu heparin bağlı PTFE greftler (%92.1) ve standart PTFE greftler (%91.4) izledi. İkinci yıl açık kalma oranları otolog ven greftler, heparin bağlı PTFE greftler ve standart PTFE greftler için sırasıyla %88.1, %84.3 ve %82.1 idi. Üçüncü yıl açık kalma oranları otolog ven greftler, heparin bağlı PTFE greftler ve standart PTFE greftler için sırasıyla %86.3, %82.1 ve %72.1 idi. Korelasyon analizi koroner arter hastalığı, kronik böbrek yetmezliği, diabetes mellitus, sigara ve tek arter run-off'unun baypas başarısızlığı için risk faktörü olduğunu gösterdi.Sonuç: Çalışma bulgularımıza göre, en iyi greft tercihi otolog ven greftler iken, heparin bağlı PTFE greftler standart PTFE greftlere kıyasla daha iyi bir seçenek gibi görünmektedir

Abstract

Objectives: This study aims to investigate whether heparin-bounded grafts are a useful alternative to autologous veins in low extremity bypasses?Patients and methods: Between January 2002 and March 2016, we retrospectively reviewed a total of 249 patients (169 males, 80 females; mean age 66.2 years; range, 55 to 73 years) who underwent above-knee femoropopliteal bypass. A heparin-bounded polytetrafluoroethylene (PTFE) graft was used in 41 patients, a standard PTFE graft in 136 patients, and a great saphenous vein graft in 72 patients. The patency rates at one, two, and third years were evaluated.Results: The patency rates at one year were the highest in the autologous vein grafts (94.3%), followed by heparin-bounded PTFE grafts (92.1%) and standard PTFE grafts (91.4%). The patency rates at two years were 88.1%, 84.3%, and 82.1% in the autologous vein grafts, heparin-bounded PTFE grafts, and standard PTFE grafts, respectively. The patency rates at three years were 86.3%, 82.1%, and 72.1% in the autologous vein grafts, heparin-bounded PTFE grafts, and standard PTFE grafts, respectively. Correlation analysis showed that coronary artery disease, chronic renal failure, diabetes mellitus, smoking, and one artery run-off were risk factors for bypass failure.Conclusion: According to our study results, autologous vein grafts are the most optimal grafts, while heparin-bounded PTFE grafts seem to be a better option than standard PTFE grafts
