Özet
Amaç: Splenik arter anevrizması, visseral anevrizmalar içinde ise en sık görülendir. Bu çalışmamızdaki amacımız, splenik arter anevrizmasına uyguladığımız cerrahi yöntemleri sunmaktırGereç ve Yöntemler: Splenik arter anevrizması olan 12 hasta çalışmaya dahil edildi. Üçüne anevrizmektomi + splenektomi, 2sine anevrizmektomi + end to end anastomoz ve 1ine otosplenektomi uygulandı.Bulgular: Hastaların yaş ortalaması 61 (50-73) yıl ve ortalama anevrizma çapı 3,1 cm (1,4-5,5 cm) idi. Hastalarda enfeksiyon, hematom veya mortalite görülmedi.Sonuç: Splenik arter anevrizması nadir görülen bir patolojidir. Splenik arter anevrizmasının mortal seyreden komplikasyonları bulunduğundan, erken tanı önemlidir. Splenik arter anevrizmasının tedavisinde splenektomi en sık kullanılan cerrahi yöntemdir. Fakat splenektomi enfeksiyon riskini arttırmaktadır. İdeal tedavi şekli splenik arterde akımı korumak ve anevrizmayı dolaşım dışı bırakmaktır.
Abstract
Objective: Splenic artery aneurysm is the most common visceral aneurysm. Our aim in this study was to provide surgical methods we used in the treatment of splenic artery aneurysms.Material and Methods: Splenic artery aneurysms of 12 patients were included in the study. Three patients underwent aneurysmectomy + splenectomy, 2 patients aneurysmectomy + end to end anastomosis and 1 patient autosplenectomy.Results: The mean age of the subjects was 61 years (range: 50-73).The mean diameter of the aneurysm was 3.1 cm (range: 1.4-5.5 cm). We did not encounter infection, hematoma, or mortality in any of our patients.Conclusion: Splenic artery aneurysm is a rare disorder. It has mortal complications, and early diagnosis is important. Splenectomy is the most commonly used treatment modality in the treatment of splenic artery aneurysms. However, splenectomy increases the risk of infection. The ideal method for the treatment of splenic artery aneurysms is to preserve flow in splenic artery and disable circulation in the aneurysm.
