Özet

Lineer sebaseöz nevus sendromu nörokütan displaziler grubundan bir hastalıktır. Major'bulguları; yüzün yarısında sebaseöz nevüsler, göz bulguları, mental gerilik ve erken dönemde başlayan konvülsiyonlardır. Belirtilen major bulguları santral sinir sistemi malformasyonları, vasküler ve ürogenital anomaliler eşlik edebilmektedir. Bu sendromda prognozu santral sinir sistemi tutulumu belirler. Vakalarda mental gerilik ve deri lezyonlarından bazal hücreli karsinom gelişme riski olduğu bildirilmektedir. Burada sunulan 19 günlük vakanın sağ supraorbital bölgede ve parietooksipital kranium derisinde, sağ üst göz kapağını da içerisine alan, unilateral, orta hattı geçmeyen, sarı turuncu renkte makülopapüler lezyonları vardı. Lezyonun histopatolojik incelemesinde sebaseöz glandlarda hiperplazi saptandı. Vakanın izleminde yüze lokalize solda fokal konvülsiyonları gelişti. EEG'de sağ hemisferde keskin dalga paroksismaları, kranial ultrasonografide lateral ventriküllerde asimetri, kranial magnetik rezonans görüntülemede ventrikül asimetrisi ile beraber fokal pakigri saptandı.

Abstract

Linear sebaceous nevus syndrome is a disease belonging to the group of neurocutaneous dysplasnas. Its major manifestations are the sebaceour nevi arising unilaterally on the face, ocular findings, mental retardation and early-onset seizurez. Central nervous system malformations or vascular and urogenital anomalies may accompany the major manifestations mentioned above. Central nervous system involvement determines the prognosis. Mental retardation and risk of development of basal cell carcinoma from skin lesions have been reported. A 19-day-old male infant presented to our outpatient clinic with facial, ocular and scalp lesions. Yellow-to-orange maculopapular lesions were present on the right supraorbital area and parieto-occipital scalp, extending to the right upper eyelid but not beyond the median line. A biopsy specimen of the lesion revealed hyperplasia in the sebaceous glands. Focal seizures confined to the left side of the face were observed during follow-up. Paroxysmal sharp waves from the right hemisphere were detected on the EEG and asymmetric lateral ventricles were seen on cranial ultrasonography. Magnetic resonance imaging showed asymmetry of the lateral ventricles and focal pachygyria.
