Özet

Bu çalışmada 1994 ve 1995 yıllarında Dokuz Eylül Üniversitesi Hastanesi Yenidoğan Yoğurt BaRiıtı Ünitesi'nde izlenen çok düşük doğum ağırlıklı (2) ve ikisinde nekrotizan enterokolit (stage 22) saptanmış ve yaşayan vakalarda, morbiditesiz sağkalım oranı % 80 olarak bulunmuştur. Çalışmada total mortalite oranı da % 14.4 olarak bulunmuştur. Vakaların doğum ağırlığına göre mortalite oranlarına bakıldığında artan doğum ağırlığıyla birlikte mortalite oranlarının azaldığı; gebelik yaşa göre mortalite oranlarına bakıldığında, 27 haftanın üzerinde sağkalım - oranlarının önemli oranda arttığı saptanmıştır. Ortalama hastanede yatış süresi yaşayan vakalar için 38.9±19.2 gün, ölen vakalar için 11.9+14.7 gün olarak saptanmıştır. Bu sonuçlar vaka sayısı az olmakla birlikte ülkemizde yenidoğan yoğun bakım şartlarının iyileşmesiyle çok düşük doğum ağırlıklı bebeklerde yüzgüldürücü sonuçlar elde edilebileceğini göstermiştir.

Abstract

This study describes the outcomes of 76 very-low-birth-weight infants who were admitted to the Neonatal Intensive Care Unit of Dokuz Eylül University Hospital in 1994 and 1995. The mean birth weight was 1235±243 g and the mean gestational age was 29.5±2.6 weeks. The birth weight and gestational age distribution showed that the majority of infants (51.5%) weighed 1251 to 1500 g, and 63 percent infants were between 28 and 31 weeks gestation. Antenatal steroids were administered to only 13 percent of mothers, and 67 percent of deliveries were vaginal. Overall, 48.6% percent of the infants hand respiratory distress syndrome, 43.4 percent of infants received surfactant, and the rate of ventilator support was 55.2 percent. Pneumothorax and other air leak syndromes were not diagnosed in any of these infants. Eight infantshad periventricular leukomalacia, three infants had intracranial hemorrhage (>grade II) and two infants had necrotizing enterocollitis (a stage II), and the rate of survival without major morbidity was 80 percent. The mortality rate was 14.4 percent. Birth-weight-specific and gestational age-specific mortality rates demonstrated that the mortality rate was decreased with increasing birth weight, and an increased survival rate was evident in infants born at a gestational age of >27 weeks. The mean hospital stay was 38.9±19.2 days for survivors and 11.9+14.7 days for infants. Although these data represent relatively small numbers, advances in neonatal intensive care units in our country will enable us to enjoy significantly improved outcomes in very-low-birth-weight infants.
