Bu çalışma Aralık 1994- Kasım 1995 tarihleri arasında Çocuk Yoğun Bakım Birimimize kritik ya da potansiyel kritik hastalık nedeniyle yatırılan 0-6 yaş grubu 140 çocukta ölüm riski ölçeğini uygulayarak ölçeğin kullanım ve yararlılığını değerlendirmek amacıyla yapılmıştır. Gözlenen ve beklenen ölüm sayıları "Lemeshow-Hosmer goodness-of-fit khi-kare testi (khi-kare uygunluk testi) ile karşılaştırıldı. Ölüm riski ölçeğinin özgünlük ve duyarlılığı "receiver operating characteristics" eğrisi (kesim noktaları tanı eğrisi) ile değerlendirildi. Abstract
In this study, we evaluated the usefulness of the "pédiatrie risk of mortality score" (PRISM) in 140 critically ill or potentially critically ill children admitted to our pédiatrie intensive care unit between December 1994 and November 1995. The study group consisted of children between zero and six years of age. The differences between the number of observed and expected deaths were tested using the chi-square goodness-of-fit test based on a modification by Lemeshow and Hosmer. We evaluated the sensitivity and specifity of the mortality risk score with the receiver operating characteristics curve. In critically ill patients with a mortality risk above 50 percent, the observed number of deaths was less than the expected. However, in potentially critically ill patients with a mortality risk below five percent, more deaths were observed than were expected. The differences were statistically significant. We demonstrated a significant difference between the observed and expected mortality rates among infants, but this difference was hot significant in children. We found that the PRISM score was more successful in predicting outcome of patients with an acute illness than with a chronic illness. We concluded that chronic disease must be added to the score as a parameter or co-factor.
