Özet

"Tethered" kord sendromunun tanısal kriterlerinden biri, spinal kordun spinal kanal içinde düşük yerleşimidir. Ancak spinal kordun yenidoğanlarda ve erken süt çocukluğundaki sonlanım düzeyi konusunda kesin bir fikir birliği yoktur. Yaygın olarak kabul edilen görüşe göre yenidoğanlarda üçüncü lomber vertebra (L3) düzeyinde olan ya da olabilen sonlanım düzeyi iki ya da üç aydan sonra L1-2 düzeyine yükselerek erişkin konumuna ulaşır. Çalışmamızda gelişimsel serebral ya da spinal anomalisi olmayan 0-2 ay arası 192 bebeğe (100 matür yenidoğan ve 30-60 gün arası 82 süt çocuğu) spinal ultrasonografi yapılarak medulla spinalisin vertebral sonlanım düzeyi saptandı. Sagittal olarak ölçülen torakal ve lomber çaplar da ayrıca değerlendirildi. Sonlanım seviyesi 0-15 günlük grupta en sık L2 (% 44) üzerindeyken, bunu az farkla L1-2 aralığı (%42) izlemekteydi; 16-30, 0-30 ve 31-60 günlük gruplarda ise en sık saptanan sonlanım düzeyi L1-2 aralığı idi. Ortalama torakal çap 3.2±0.3 mm, lomber çap 5.0±0.4 mm olarak saptandı. Sonuç olarak konuş medullarisin 0-2 ay arasında en sık L1-2 aralığında sonlandığı, yaşamın ilk onbeş günü için saptanan düşük yerleşimin pratikte önemsenmeyecek kadar küçük olduğu ve konusun erişkin düzeyine yaşamın ilk günlerinde zaten ulaştığı bulundu.

Abstract

The low-lying conus medullaris is one of the key features of the tethered cord syndrome, therefore the vertebral level of termination of the spinal cord must be well-known throughout childhood. There are some variations in the definition of the normal level of termination of the spinal cord in the newborn and early infancy periods. It has classically been thought that the normal conus is located at lumbar (L) 3 vertebral body in newborns, rising to the adult level of L1-2 at two months of age. In the present study, 192 babies (110 full-term newborns and 82 infants aged 30-60 days) with no cerebral or spinal anomaly were included and the conus levels were detected by spinal ultrasonography. Sagittal diameters of the thoracic and lumbar spinal cord were also evaluated. In the 0-15 days of age group the commonest termination level of the conus was L2 (44%) followed by L1-2 (42%). In the 16-30, 0-30 and 31-60 days of age groups, the termination point was the L1-2 disc space. The mean sagittal diameters of the thoracic and lumbar spinal cord were respectively 3.2+0.3 mm and 5.0±0.4, respectively. In conclusion, the termination level of the conus was L1-2 during the first two months of life. Although there was a significant but slight rise in conus position between babies aged 0-15 days and the others, this difference could be ignored in practice.
