Çocuklarda akut astma nöbetlerindeki başlıca etmenlerden birisi olarak Chlamydia pneumoniae enfeksiyonu

Özet
Bu çalışmada akut astma nöbeti ile acil servise getirilen çocuklarda, C. pneumoniae enfeksiyonunun yaygınlığı ve bu enfeksiyonu geçiren astmalr çocukların klinik ve laboratuvar özellikleri araştırılmıştır. Çalışmada yaşları 30-156 ay arasında değişen, daha önceden astmalı olduğu bilinen 21 'i erkek, dokuzu kız 30 çocuk hasta ile benzer yaş ve cinsiyette, hiçbir solunum sistemi yakınması olmayan 20 sağlıklı çocuk incelenmiştir. Hastaların altısında (% 20) orta şiddette, 24'ünde (% 80) hafif şiddette akut astma nöbeti vardı. Hastaların klinik ve laboratuvar özellikleri kaydedildi, iki hafta ara ile alınan boğaz sürüntü örnekleri değişik besiyerlerine ekildi. Hastalardan akut nöbette ve bir ay sonra, kontrol grubunda ise aynı dönemlerde bir ay ara ile alınan çift serum örneği çalışma anına kadar -20 "C'de saklandı. C. pneumoniae IgM ve IgG antikorları mikroimmünofloresan (MIF) yöntemi ile, M. pneumoniae IgM vé IgG antikorları ise kompleman fiksasyôn testi ile araştırıldı. Çalışmada sekiz hastada (% 26.6) akut C. pneumoniae enfeksiyonu saptanırken, kontrol grubundaki çocukların hiçbirinde akut C. pneumoniae enfeksiyonu saptanmadı (p0.05). Bu çalışmanın sonuçları, astmalı çocuklarda akut atakların gelişmesinde C. pneumoniae enfeksiyonunun önemli bir rol oynadığını düşündürmektedir.
Abstract
In this study, the prevalence of C. pneumoniae infection is asthmatic children presenting at the pédiatrie emergency department with an acute episode of asthmawas determined. Also, the clinical and laboratory features of the asthmatic children with this infection were investigated. Thirty asthmatic patients (21 males and nine females, aged between 30-156 months) and 20 age-and-sex matched healthy controls were included in this study. Out of the total patients, 6 (20%) had moderate and 24 (80%) had mild acute episodes of asthma. Clinical and laboratory features of the patients were recorded and throat cultures were taken twice, with an interval two weeks. Paired serum samples for determination of antibodies to C. pneumoniae and Mycoplasma pneumoniae were collected and frozen at -20 °C until tested. Sérologie studies were performed with the microimmunofluorescence method against the prototype strain TW183 using anti-IgG and anti-IgM conjugates for C. pneumoniae, and with the complement fixation method using anti-IgG and anti-IgM conjugates for M. pneumoniae.
