Özet

Down sendromu Hirschsprung hastalığı ile birlikteliği sıklıkla bildirilmektedir. On yedi yıllık süre içerisinde izlenen 302 vakalık Hirschsprung hastalığı serisinde beş hastada Hirschsprung Hastalığı ve Down sendromu birlikteliği saptanmıştır (%1.7). Hastaların beşinin de iki aydan küçük olduğu, erkek kız oranının 4:1 olduğu gözlenmiştir. Başvurma şekli ve geliş belirti ve bulguları arasında intestinal obstrüksiyon, ateş, kabızlık ve dehidratasyon bulunmaktaydı. Tümünde mekonyum çıkışı ikinci günden sonraydı. Sadece birinde ek anomali vardı. Bu grupta enterokolit gözlenmedi. Bu hasta grubunda mortalite %80 bulundu. Sonuçlarımız Hirschsprung hastalığı ve Down sendromu birlikteliğinin enterokolit olmadan da mortalitesi yüksek bir ilişki olduğunu düşündürmektedir

Abstract

Down syndrome is reported in association with Hirschsprung's disease with increasing frequency. In a series of 302 Hirschsprung patients over a period of 17 years, five patients with Down syndrome and Hirschsprung's disease association were detected (1.7%). All five patients were below two months of age. The male :female ratio was 4:1. Among presenting signs and symptoms and mode of presentation were intestinal obstruction, fever, constipation and dehydration. All patients passed meconium after the second day of life. Only one had an associated anomaly. No enterocolitis was detected in this group. Mortality was 80 percent among these patients. These data suggest that the course of Hirschsprung's disease among patients with Down syndrome is more fatal compared to other associations, even in the absence of enterocolitis.
