Hipoksik iskemik ensefalopatili yenidoğanlarda prognozun Denver II gelişimsel tarama testi ile değerlendirilmesi

Özet

Bu çalışma Çukurova Üniversitesi Tıp Fakültesi Yenidoğan Ünitesi'nde hipoksik iskemik ensefalopati (HİE) tanısı ile izlenen 52 hastanın uzun dönem prognozlarmı ve prognozu, etkileyen perinatal faktörleri incelemek amacıyla yapıldı. Dosyaları inceledikten sonra Denver II-GTT uygulamak ve nörolojik durumlarını değerlendirmek üzere hastalar yeniden çağrıldı. Hastaneye yatış sırasındaki klinik durumları Sarnat ve Sarnat klasifikasyonuna göre üç evreye ayrıldı. Buna göre %23'ü evre I'de, %67.3'ü evre II'de, %9.6'sı ise evre III'de idi. Evre I'deki hastaların tümünde nörolojik muayene ve Denver II-GTT normal bulunurken, evre III'deki hastaların tümünde anormal, evre Il'de ise %65.7 oranında anormaldi. Hastalar sekel yönünden değerlendirildiğinde evre I'deki hastaların tümünün sekelsiz olduğu, evre III'deki hastaların tümünde sekel geliştiği, evre H'de ise %22.8 oranında ağır, %25.7 oranında orta ve %17.1 oranında ılımlı sekel geliştiği saptandı. Hastalarda en çok rastlanan sekeller serebral palsi, mental retardasyon, epilepsi, mikrosefali, işitme ve görme kaybı idi. İleri klinik evrenin, kız cinsiyetin ve öyküde konvülsiyon olmasının prognozu olumsuz yönde etkilediği (p0.05). Sonuç olarak, HİE'li hastaların uzun dönem prognozlarının, yenidoğan dönemindeki klinik bulguların ağırlığı ile doğrudan ilişkili olduğu ve klinik evre yükseldikçe prognozun daha kötü olduğu saptandı.

Abstract

Fifty-two patients diagnosed as hypoxic-ischemic encephalopathy (HIE) and admitted to Çukurova University Faculty of Medicine, Neonatal Intensive Care Unit, were studied in order to determine the long-term prognosis and perinatal factors affecting outcome. The charts of the patients were reviewed. The children were reexamined using Denver II Developmental Screening Test (Denver II-DST) and by neurological evaluation. The clinical status of the patients was graded using the staging system of Sarnat and Sarnat into three stages. Twenty-three percent of the infants were in stage I, 67.3% were in stage II and 9.6% were in stage III. Neurological examination and Denver II-DST of all infants in stage I were normal and in stage III were abnormal. In stage II, 67.3% of them had abnormal results; 22.8% had severe, 25.7% had moderate and 17.1% had mild sequelae. The sequelae included cerebral palsy, mental retardation, epilepsy, microcephaly, deafness and blindness. Psychomotor retardation, female gender and presence of seizure were poor prognostic factors (p0.05). In conclusion, we found that the long-term prognosis of infants with HIE was predicted by the clinical status in the newborn period, and that clinical stage and prognosis significantly correlate with each other.
