Prematürelerde Ballard ve yeni Ballard Skorlama Yöntemleri ile belirlenen gebelik yaşlarının karşılaştırılması

Özet
İntrauterin büyümenin sağlıklı olarak değerlendirilebilmesi için gebelik yaşının tam olarak bilinmesi gereklidir. Gebelik yaşının belirlenmesinde son adet tarihine göre hesaplama (SAT), antenatal erken ultrasonografik değerlendirme (USG) yanında fizik ve nöromusküler muayene bulgularına dayanan skorlama, gebelik yaşının belirlenmesinde sıklıkla kullanılan yöntemlerdir. Bu amaçla kullanılan Ballard skorunun parametreleri daha küçük prematürelere uyarlanarak Yeni Ballard skoru geliştirilmiştir. Bu çalışmada prematürelerin gebelik yaşının belirlenmesinde Ballard ve Yeni Ballard yöntemleri kullanılarak belirlenen gebelik yaşları arasında fark olup olmadığı ayrıca bu skorlar ile SAT ve USG ile belirlenen gebelik yaşları arasındaki farkın değerlendirilmesi amaçlandı. Yenidoğan Yoğun Bakım Unitesi'nde izlenen USG ile gebelik yaşları belirlenmiş doğum ağırlığı 750-2730 gr arasında (1892 ± 560 gr) toplam 100 prematüre yenidoğana SAT ve USG'ye göre belirlenen gebelik haftasından habersiz iki neonatoloji yandal uzmanlık öğrencisi tarafından Ballard ve Yeni Ballard skoru uygulandı. Sonuçlar gebelik haftası 34 hafta ve altında (n=60); ve 34 haftanın üstündeki prematüreler (n=40) olmak üzere iki gruba ayrılarak değerlendirildi. Abstract
For exact evaluation of intrauterine growth, it is necessary to know gestational age. The methods frequently used for calculation of the gestational age are according to the last menstral period, early antenatal ultrasonography (USG), and physical and neuromuscular evaluation. The parameters of the Ballard score used for this purpose have been expanded for smaller premature infants, and the improved new score is called New Ballard Score. The aim of this study was to compare the gestational ages determined by Ballard and New Ballard scores, and to determine the differences ascertained by these methods, and as calculated according to the last menstral period or by early antenatal ultrasonography. One hundred premature infants weighing between 750 and 2730 g followed in the neonatal intensive care unit whose gestational ages were determined by ultrasonography were included in the study. The Ballard and New Ballard scores were applied to these infants by two Neonatology fellows. The results were evaluated by dividing the patients into two groups according to their gestational age: $leq$ 34 weeks (n=60) and >34 weeks (n=40). No difference was found in the gestational age of the older premature infants (>34 weeks) determined by the Ballard and New Ballard scores. In smaller premature infants ($leq$ 34 weeks), however, the gestational age determined by the Ballard score was greater than that determined by both USG and the New Ballard score. Further more, the gestational ages determined by the New Ballard score were closer to the gestational ages determined by USG than were those determined by the Ballard score. Additionally it has been found that antenatal steroid usage does not make any difference in determination of gestational age. In conclusion we think that the New Ballard score is more useful in determining gestational ages, especially of smaller premature infants.
