Fototerapi ve indirekt hiperbilirübineminin bazı neonatal immün sistem parametrelerine etkileri

Özet
Bu çalışma neonatal hiperbilirübinemi ve fototerapinin neonatal hematopoetik ve immün sistemin bazı komponentlerine etkilerini incelemek amacıyla zamanında doğmuş ve normal doğum ağırlıklı 77 yenidoğanda yapıldı. Bunların 30'u indirekt bilirubin düzeyi 15 mg/dl'den yüksek üç ile 10 gün arası bebekler olup 72 saatlik fototerapi öncesi grup A, fototerapi sonrası grup B olarak belirlendi. İndirekt bilirubin düzeyleri 10 mg/dl'den düşük, yaş, ağırlık ve cinsiyetleri grup A'ya benzer olan 26 yenidoğan A grubunun kontrolü (grup C); yaş, ağırlık, cinsiyet ve bilirubin düzeyleri B grubuna benzeyen 21 yenidoğan da B grubunun kontrolü (grup D) olarak belirlendi. Hiperbilirübinemik vakalardan fototerapi öncesi ( A grubu) ve fototerapi sonrası (B grubu), kontrol gruplarından çalışmaya alınma anında kan örnekleri alındı. Kan örneklerinden tam kan sayımı, nötrofil, lenfosit ve retikülosit sayımı, C3, C4, IgG, IgA ve IgM düzeyleri ve CD3, CD4, CD8, CD14, CD19, CD3/HLA-DR, CD45, ve CD16+56 yüzdeleri ölçüldü ve CD,4/CD8 oranları hesaplandı. Hiperbilirübinemik olan A grubu vakalarında CD8+ T hücre yüzdeleri diğer gruplara göre düşük (bir vakada normalden düşük), CD4CD8 oranı ise yüksek bulundu (bir vakada normalden yüksek). Fototerapi alan B grubunda ise mutlak nötrofil sayısı ve IgG düzeyleri diğer gruplara göre anlamlı derecede düşük (IgG düzeyleri beş vakada normalden düşük) bulundu. Diğer parametreler açışından gruplar arasında anlamlı farklılık yoktu. Bu bulgular hiperbilirübinemi ve fototerapinin neonatal immün sistemi etkileyebileceğini, bu yüzden dikkatli izlem ve tedavinin gerekli olduğunu düşündürmektedir.
Abstract
This study was carried out to examine the effects of hyperbilirubinemia and phototherapy on some components of the neonatal hematopoietic and immune systems. This study was performed in 77 term infants with normal weight: 30 with indirect bilirubin levels higher than 15 mg/dl, aged 3-10 days, before (group A), and after (group B) 72-hour phototherapy, 26 with indirect bilirubin levels lower than 10 mg/dl whose ages, weight and sex were comparable to group A similar to those of group B (group C= control of group A), and 21 with ages, weight, sex and bilirubin levels (group D= control of group B). Blood samples were taken before (group A), and after (group B) phototherapy from hyperbilirubinemic patients, and at the time of studying from controls (groups C and D). Complete blood count, neutrophils, lymphocytes and reticulocyte counts, C3, C4, IgG, IgA and IgM levels, expression of CD3, CD4, CD8, CD14, CD19, CD3/HLA-DR, CD45 and CD16+56 CD4/CD8 ratios were measured from blood samples. The mean percentage of CD8+ T cells in group A with hyperbilirubinemia was lower than that in the other groups (that of 1 case in group A was lower than normal limits), and mean CD4/CD8 ratios of group A cases were higher than those of the other groups (that of 1 case in group A was higher than normal limits). Mean absolute neutrophil count and IgG level of group B who underwent phototherapy were lower than those of the other groups (IgG levels of five cases in group B were lower than normal limits). There were no significant differences among the groups regarding the other parameters. These findings suggest that unconjugated hyperbilirubinemia and phototherapy may affect the neonatal immune system, and careful observation and treatment are required.
