Meningoencephalitis in the course of acute viral hepatitis: A report of two cases

Özet

Akut viral hepatit A çocuklarda hafif seyrine rağmen pek çok organ ve sistemi ilgilendiren komplikasyonlara yol açabilir. Hepatik ensefalopati dışında akut viral hepatit A seyri sırasında nörolojik bulgular oldukça nadirdir. Kliniğimizde akut viral hepatit A seyri sırasında görülen, birisi konvülziyon ile gelen iki meningoensefalit vakası sunulmuştur. Hastalarda akut viral hepatit A'nın serolojik, klinik ve biyokimyasal bulguları yanında, meningeal irritasyon bulgularının varlığı, beyin-omurilik sıvısında pleositoz olması, elektro-ensefalografi bulguları ile meningoensefalit tanısı konuldu. Her ikisi de kısa sürede düzeldi. Akut viral hepatit A seyrinde meningoensefalitin seyrek görülmesi ve meningoensefalit vakalarında hepatit A virüsünün daha sık düşünülmesi gerektiğini vurgulamak için vakalar sunuldu.

Abstract

Even though acute viral hepatitis a has a mild course in childhood, many complications of different organs and/or systems may be observed. Central nervous system involvement, with the exception of hepatic encephalopathy, is very rare in acute hepatitis A. Here we report two cases of hepatitis A and meningoencephalitis. In addition to the serological, clinical and biochemical signs of acute hepatitis A, the patients had meningeal irritation signs, cerebrospinal fluid pleocytosis and EEC findings consistent with meningoencephalitis. Their conditions improved quickly in follow-up. We report these cases because of the rarity of the association of hepatitis A and meningoencephalitis and we stress that hepatitis A must be kept in mind in the etiology of meningoencephalitis.
