Transhiatal özofajektomiden sonra morbidite ve mortaliteyi etkileyen faktörler

Özet
Özofajektomi, morbidité ve mortalitesi elektif ameliyatlar arasında en yüksek olanıdır. Günümüzde amaç özofajektomiden sonra mortaliteyi %.10'un altına indirmektir. Transhiatal özofajektominin diğer özofajektomi tekniklerine göre avantajları mevcuttur. Bu çalışmamızın amacı transhiatal özofajektomi ile opere ettiğimiz 37 özofagus kanserli olgumuzda morbidite ve mortaliteyi etkileyen faktörleri araştırmaktır. Olgularımızın 10'u servikal, 27'si torasik özofagus kanseri nedeniyle opere edildi. Hastalarımızın 13'ü kadın, 24'ü erkek, yaş ortalaması 57,5 ± 15 (22-81) idi. 3 olguda adenokarsinom, 34 olguda epidermoid karsinom mevcuttu. Özofageal substitue olarak 35 olguda mide, 2 olguda kolon kullanıldı. Olgularımızın 18'inde komplikasyon gelişti (% 48,6). Bir olguda dalak yaralanması, 3 olguda pnömotoraks, 6 olguda pulmoner komplikasyonlar (pnömoni, atelektazi, ARDS), 2 olguda plevral epanşman, 1 olguda şilotoraks, 1 olguda AMI, 2 olguda n. recurrens yaralanmasına bağlı ses kısıklığı, 2 olguda anastomoz kaçağı oluştu. Postop 30 günlük dönem dahil, hastane mortalitesi % 16,2'dir. Mortalite 2 olguda ARDS, 2 olguda pnömoni, 1 olguda AMI, 1 olguda anastomoz kaçağı nedeniyle oluştu. Preoperatif, intraoperatif ve postoperatif toplam 17 parametre morbiditeye; 18 parametre ise mortaliteye etkili faktörler olarak istatistiksel analize tabi tutuldu. Abstract
Esophagectomy has the highest mortality and morbidity among the elective operations. Today, the aim is to reduce the mortality to a level below 10 % after esophagectomy procedures. Transhiatal esophagectomy has some advantages if compared the other esophagectomy techniques. The purpose of this study is to investigate the factors affecting morbidity and mortality in 37 esophagus cancer patients who were operated with transhiatal esophagecto-my technique. Ten of our patients were operated for cervical and 27 of them for thoracic esophageal cancer. In our series, Thir-teen of patients were vomen, 24 of were ' mean and the average age was 57,54 ± 15 years (22-81) Three patients had adenocarcinoma whereas, 34 patients were epidermoid cancer. We used the stomach in 35 cases and the colon in two patients as esophageal substitue. In 18 of patients, complications were seen (% 48,6) In one patient splenic trauma; in three patients pneumothorax, in six patients pulmonary complications (pneumonia, atelectasia, ARDS), in 2 cases pleural epanchment, in one case chilothorax, in one patient AMI, in 2 cases hoarseness due to n. recurrens injury and in two cases leakage of anastomosis were observed. The mortality rate including postop 30-day period was 16.2 %. Mortality was due to ARDS in two cases, pneumonia in two patients, AMI in one patient case and leakage of anastomosis in one patient. Preoperative, operative and postoperative 17 parameters effecting morbidity and 18 paremeters effecting mortality were analyzed statistically.
