İntraperitoneal paklitaksel' in kolon anastomozunun iyileşmesine etkisi

Özet

Antineoplastik ilaçlar, intraperitoneal yolla, cerrahi girişim sonrasındaki tümör implantasyonunun önlenmesi veya peritoneal karsinomatozisin tedavisi amacıyla kullanılmaktadır. Paklitakselin intraperitoneal uygulanmasının etkinliği, antianjiyogenetik etkisinin de olduğu ileri sürülen bu ilacın, periton yüzeyinden geç emilmesi ve karaciğerde metabolize olması özelliklerinden dolayı son senelerde araştırılmaktadır. Deneysel çalışmamızda, antianjiyogenetik etkisi de gözönüne alındığında, intraperitoneal olarak cerrahi girişim sırasında ve tek doz uygulanan paklitakselin uç-uca yapılan kolon anastomozunun iyileşmesine olan etkisini belirlemeyi amaçladık. Her biri 7 sıçan içeren üç grup oluşturuldu. Grup A=kontrol grubu olarak çalışıldı. Grup BI=2.5 mg/kg ve Grup B2=3.5 mg/kg paklitaksel'in intraperitoneal olarak uygulandığı gruplar olarak belirtendiler. Tüm gruplarda postoperatif 7.günde alınan örnekler anastomoz patlama basıncı, hidroksiprolin düzeyi ve özellikle vaskülarizasyon, fibroblastik aktivite ve kollajen yoğunluğunun araştırıldığı histolojik inceleme ile değerlendirildiler. İncelenen tüm parametreler B1 ve B2 grubunda kontrol grubuna göre azalma gösterdiler. B1 grubundaki azalmalar, kontrol ve B2 grubuna göre istatistiksel olarak anlamlılık göstermediler. Abstract

Antineoplastic drugs have been used for the prophylaxis of the implantation of tumor cells following surgical procedures or the treatment of peritoneal carcinomatosis by intraperitoneal route. Recently, the antineoplastic potential of paclitaxel which also possesses the antiangiogenetic effect is investigated for intraperitoneal administration because of its low clearance from peritoneal cavity and to metabolize in the liver. In our experimental study, by taking into consideration its angiogenetic effect, we aimed to evaluate the effect of paclitaxel administrated by one-time and intraperitoneal during operation on the healing of end-to-end colonic anastomosis. Three groups were constituted that each of them included 7 rats. Group A was control. B1 and B2 were groups of paclitaxel administrated by intraperitoneal in the doses of 2.5 mg/kg and 3.5 mg/kg respectively. In all groups, the measurements of bursting pressure, hydroxyproline contents, and histologic evaluation of anastomoses were studied on 7th day after operation. All parametres showed a decrease in both of B1 and B2 groups when compaired with Group A. The measurements of group B1 were not statistically significant compared with other groups.
