Kısa süreli total parenteral nütrisyon uygulamasında oluşan metabolik değişiklikler

Özet

Total parenteral nutrisyonun (TPN) perioperatif dönemde mortalite üzerine etkisi olmasa da beslenme bozukluğu olan hastalarda morbidite oranını azalttığı kabul edilmektedir. Uzun süre TPN uygulanan hastalarda osteomalasi ile karakterize metabolik kemik hastalığı gelişmektedir. Kısa süreli TPN uygulanan hasta grubunda metabolik kemik hastalığı gelişimi konusunda fazla bilgi yoktur. Çalışmanın amacı kısa süreli TPN uygulamalarında kalsiyum metabolizması ile ilgili değişiklikleri araştırmaktır. TPN solüsyonuna kalsiyum ilave edilen (A grubu) ve edilmeyen (B grubu) iki hasta grubu oluşturuldu. Uygulama öncesi ve sonrası hastaların ağırlık, kan total protein, albumin, serum glutamik pirüvik transaminaz (ALT), serum glutamik oksaloasetik transaminaz (AST), alkalen fosfataz (ALP), parathormon, kalsitonin, kan ve idrar kalsiyum, fosfor değerlerine bakıldı. TPN gereksinimi Harris-Benedict denklemine göre hesaplandı. İstatistiki değerlendirmede grup içi karşılaştırmada paired-t testi, gruplar arası karşılaştırmada unpaired-t testi kullanıldı. TPN uygulama, süresi, A grubunda ortalama 9.8 gün, B grubunda 10.3 gündü. TPN uygulama sonrası kilo alma, kan proteinlerinde artış gibi olumlu etkiler yanında; AST, ALT, ALP değerlerinde yükselme ortaya çıkabilmektedir. Kalsiyum metabolizması açısından B grubunda normal laboratuar değerleri arasında kalan bir kan kalsiyum düşmesi olurken bunu kompanze etmek üzere kan parathormon değerlerinde artış, kalsitonin değerlerinde düşme, idrar kalsiyumunda azalma ve idrar fosfor değerlerinde artış olmaktadır. Yakın takiple 10 günden kısa süreli TPN uygulamalarında TPN solüsyonuna kalsiyum ilave edilmesine gerek olmadığını düşünüyoruz.

Abstract

Although perioperative TPN therapy has no effect on the mortality rate, it is assumed that TPN decreases the morbidity risk rate in malabsorbtive patients. Long term TPN therapy can cause some metabolic bone diseases, such as osteomalasia. There is no information about the effect of short term TPN therapy on the development of metabolic hone disorders. The aim of clinical study is to investigate role of short term TPN therapy on the calcium metabolism. The study consist of two groups. The study group contains the patients taking supplementary calcium in the TPN solution. The control group is supplied by TPN without calcium. Blood plasma levels of total protein, albumin, ALT, AST, ALP, parathyroid hormone, calcitonin, calcium and phosphorus are detected before the beginning and at the end of TPN therapy. Patients weight is also recorded. The needed amount of TPN solution is estimated by the help of Harris-Benedict formula. Statistical analysis is performed using the paired-t test and unpaired-t test for the comparison of the group and the inter group values, respectively. Mean TPN therapy duration is 9,8 and 10,3 days in the study and control group, respectively. After the treatment with TPN solution, the increase in the weight and plasma protein concentration levels can be detected. Increase in the level of AST, ALT, ALP, although not much, can be detected. Decrease in calcium level within the normal ranges is detected in group B; at the same time compensatory increases in the levels of parathyroid hormones and urinary phosphorus, decreases in the levels of calcitonin and urinary calcium are detected. As a result we assume that short term (10 days) TPN solution therapy application does not need calcium supplements.
