Pankreas karsinomlarında rezektabilitenin ultrasonografi, bilgisayarlı tomografi ve anjiyografi ile değerlendirilmesi

Özet

Bu retrospektif çalışmada, pankreas karsinomlarında tümör rezektabilitesinin belirlenmesinde ultrasonografi (US), bilgisayarlı tomografi (BT) ve anjiografinin güvenirliğinin karşılaştırılması amaçlanmıştır. Ocak 1993 - Nisan 1999 tarihleri arasında kliniğimize başvuran 28 pankreas karsinomlu hasta çalışmaya dahil edilmiştir. Rezektabilitenin peroperatif tespitinde pankreas cerrahisinde deneyimli cerrah tarafından, peroperatif sitoloji, büyük damar invazyonu (sol gastrik arter, superior mezenterik ven, inferior kaval ven ve portal ven) lokal yayılım (retroperiton, mezokolon ve diğer dokular), bölgesel lenfatik yayılım ve uzak metastaz (karaciğer, periton ve diğer dokular) dikkate alınmıştır. Preoperatif dönemde yapılan US, BT ve anjiyografi değerlendirmeleri, peroperatif bulgularla karşılaştırılmıştır. Çalışmaya alınan 28 hastanın 6'sına, uzak organ metastazı tespit edilmesi nedeniyle sadece internal stent yerleştirilmiş, herhangi bir cerrahi girişim uygulanmamıştır. Preoperatif çalışmada hastaların tümüne US ve BT, 25'ine anjiografi yapılmıştır. Yapılan 28 US'den 20'si (%71.4), 28 BT'den 24'ü (% 85.7) pankreas karsinomu açısından tanısal sonuç vermiştir. US ile tümör tespit edilen 20 hastadan 14'üne preoperatif dönemde rezektabl tanısı konulduğu halde, intraaperatif olarak 6'sı rezektabl olarak bulunmuştur. BT'de tümör tespit edilen 24 hastanın 7' si preoperatif rezektabl olarak değerlendirilmiş, bu hastaların peroperatif olarak 4'ünün rezektabl olduğu belirlenmiştir. Anjiografi yapılan 25 hastanın ise 10'unda preoperatif olarak rezektabilite tespit edilmiş, hastaların 7'sinin peroperatif olarak rezektabl olduğu gözlenmiştir. Bu sonuçlar doğrultusunda sensitivite, spesifisite, PPV (pozitif prediktif değer), NPV (negatif prediktif değer) sırasıyla US için % 85.71, % 38.48, % 42.85, % 83,33, BT için % 50.00, % 81.25, % 57.14, % 76.47, anjiyografi için ise % 77.77, % 81.25, % 70.00, % 86.66 olarak bulunmuştur. Pankreas kanserlerinin definitif cerrahi tedavisi öncesinde anjiografi ve BT değerlendirmesi önemlidir.

Abstract

The aim of this retrospective study is to investigate the sensitivity, specificity and predictive value of ultrasonography (US), computerized tomography (CT) and angiography in determining the resectability of pancreatic carcinomas. This study includes 28 patients with pancreas carcinoma admitted to our clinic between January 1993 and April 1999. The preoperative criterias for unresectability are evaluated by an experienced surgeon as: (1) Major vascular invasion (left gastric artery, superior mesenteric vein, inferior vena cava and portal vein) (2) Local invasion (retroperitoneum, mesocolon and other structures) (3) Regional lymphatic spread and distant metastases (liver, peritoneum and other structures) As 6 of the 28 patients had distant organ metastases an internal stent was placed or no operations was performed. All the patients were examined with US and CT preoperatively and 25 with angiography. 20 of the US (71.4 %) and 24 of CT (85.7 %) were diagnostic for pancreatic carcinoma. 14 of the 20 patients diagnosed with US were regarded as resectable preoperatively but 6 were found to he resectable intraoperatively. 7 of the 24 patients diagnosed with CT were regarded as resectable preoperatively but 4 of them were found to be resectable peroperatively. Of the 25 patients examined with angiography, 10 were predicted to be resectable. 7 of this patients were resectable peroperatively. So the sensitivity, specificity, positive predictive value and negative predictive value are found to be ; US, 85.71 %, 38.48 %, 42.85 %, 83.33 %, CT 50.00 %, 81.25 %, 57.14 %, 76.47 %, Angiography 77.77 %, 81.25 %, 70.00%, 86.66 % respectively. Angiography and CT are important in the management of definitive surgical treatment of the pancreatic carcinomas.
