Koledok taşının tedavisinde transduodenal sfinkteroplasti

Özet

Transduodenal sfinkteroplasti koledok taşının tedavisinde kullanılan yöntemlerden biridir. Geçen 10 yıl içinde 48 koledok taşlı hasta bu yöntemle tedavi edildi. Başlıca sfinkteroplasti indikasyonları 20 olguda multipl koledok taşı ve çamur; 11 hastada papilla vateri'ye sıkışmış taş; 8 vakada multipl taş ve Oddi sfinkteri darlığı; 6 olguda kolesistektomi sonrası unutulmuş veya nüks etmiş taş; 2 hastada sump sendromu ve bir kişide intrahepatik kanal ve koledok taşı bulunmasıdır. Ameliyat sonrası hiçbir hasta kaybedilmedi 5 hastada (%10.4) önemsiz komplikasyonlar görüldü. 48 hastadan 35'i izlenebildi. Bunların 33'ünden çok iyi sonuç alındı. En önemli geç dönem komplikasyonu iki hastada oluşan sfinkteroplasti darlığıdır.

Abstract

Transduodenal sphincteroplasty have, been used in the treatment of the common bile duct stones (CBD). During the past 10 years these methods were performed to 48 cases (30 Female, 18 Male). The main indications for sphincterotomy were multipl CBD stones and sludge (20); multipl CBD calculi and stenosis of Oddi (8); impacted stones in papilla Vateri (11); primary or retained Choledocholithiasis (6); sump syndrome after choledochoduodenostomy (2); and intrahepatic canal and CBD stones (one). In this series the mortality rate was nil. Various complications were seen in five (10.4 %) patients. 35 of the 48 patients could be followed-up. 33 of the 35 cases in whom follow-up was obtained, an excellent or good results were achieved. The late important complications were the stenosis of the Sphincteroplasty (two cases).
