Abdominal duvar defektlerinin primer ve sentetik greft ile onarımı (deneysel çalışma)

Özet
Abdominal cerrahi sonrasında oluşan abdominal duvak defektlerinin ve insizyonel hernilerin onarımı güncel cerrahi uygulamada halen ciddi bir problemdir. Çalışmada, sıçanlarda deneysel olarak oluşturulan abdominal duvar defektlerinin onarımında primer onarım, polipropilen (PP) ve politetrafluoroetilen (e-PTFE) greft ile onarım yöntemlerinin karşılaştırmalı olarak değerlendirilmesi amaçlanmaktadır. Otuz adet sıçanın karın ön duvarlarından 4x2 cm'lik kısım cilt hariç, adale ve periton ile birlikte çıkarıldı. Defektin 113 üst kısmı PP greft, 1/3 orta kısmı primer, 1/3 alt kısmı e-PTFE greft kullanılarak onarıldı. Sıçanlar rastlantısal olarak 10 ar sıçandan oluşan üç gruba ayrıldı. Birinci grup postoperatif 20nci gün, II.grup postoperatif 40. gün, III. grup postoperatif 80 inci gün sakrifiye edilerek batın ön duvarları bir bütün olarak çıkarıldı. Primer ve greftlerle onarılan üç bölgeden üç ayrı doku parçası hazırlandı. Parçalardan birisi Mason Trikrom ve hemotoksilen&eozin boyasıyla boyanıp histopatolojik inceleme, diğerinde doku hidroksiprolin ölçümü, son doku parçasında ise gerilim kuvveti ölçümü yapıldı. Histopatolojik olarak PP greftin oluşturduğu skar dokusunda inflamatuar hücre yoğunluğunun diğer onarımlara göre anlamlı olarak arttığı, fibroblastların greft kullanılan onarımlarda belirginleştiği, kollajen diziliminin ise e-PTFE greftte diğer onarım yöntemlerine göre daha düzenli ve ince olduğu saptandı. Visseral adezyon ve infeksiyona eğilim PP greftte e-PTFE grefte göre daha fazla oranda görüldü. Gerilim kuvveti, greft kullanılan bölgelerde primer onarım yapılan bölgeye göre daha anlamlı olarak arttı. Hidroksiprolin değerleri ise zamana bağlı artarken, mikroskopik bulgularla paralellik göstermedi. Greftle onarınım primer onarıma göre daha fazla mekanik dayanıklılık sağladığı, e-PTFE greft daha ideal özelliklere sahip olmasına rağmen, PP greftin e-PTFE grefte nazaran çok daha ucuz olması ve e-PTFE greftle arasında mekanik dayanıklılık açısından anlamlı bir fark bulunmaması nedeniyle uygun olgularda tercih edilebileceği kanısına varıldı.
Abstract
The repair of abdominal wall hernia and abdominal wall defects still continue to be a serious problem in current surgical practice. The aim of this study is to compare different abdominal wall repair techniques such as primary repair, polypropylene (PP) graft repair, and polytetrafluoroethylene (e-PTFE) graft repair in experimentally induced abdominal wall defects of rats. An abdominal wall defect of 4x2 cm including muscle and peritoneum, leaving skin intact was created in thirty rats. Upper 113 part of defect was repaired by PP graft, middle 113 by primary repair, and lower 1/3 by e-PTFE graft. Then rats were separated into three groups of 10 rats. Rats were sacrificed 20th day postoperatively in the first group, 40th day in second, and 80th day in third group and abdominal wall was resected en bloc. Three separate tissue pieces from each different repair parts were taken. One tissue piece of each part was stained by Mason's tricrom and hemotoksylen&eosin for histopathological evaluation, second piece was frozen at -30°C for hydroxyproline measurement, and last piece was used for tissue tension strength measurement. Histopathologically, inflammatory cell density of PP scar tissue was significantly higher than that of others, and fibroblasts were more obvious in graft-repaired scars. Collagen arrangements in e-PTFE graft were more regular and fine than that of others. Visceral adhesions and tendency to infection were higher in PP graft than those of e-PTFE graft. Tissue tension strengths in graft-repaired parts were significantly higher than that of primary repair. Although the tissue hydroxyproline levels increased in time, it was not correlated with microscopic results. Graft-repair provides more mechanical resistance than primary repair. Although the e-PTFE graft has more ideal features, PP graft may be preferred because it is cheaper and there is no significant difference between two meshes in the aspect of mechanical resistance.
