Özet
Son yıllarda, genel cerrahlar arasında, cerrahi hastalığı araştırmak ve tedavi etmek için girişimsel yöntemlerin kullanılması konusunda büyük bir ilgi vardır. Bu yazımızda, 12 'sinde nedenin malignite olduğu 13 tıkanma sarılıklı hastanın tedavisi için gerçekleştirdiğimiz perkütan transhepatik biliyer drenaj (PTBD) uygulamalarımızla ilgili edindiğimiz ilk deneyimin sonuçları sunulmaktadır. Bu hastalara aynı cerrah tarafından 14 adet (8'i fluoroskopi, 6'sı ultrasonografi rehberliğinde) PTBD girişimi uygulanmıştır. Bu girişimler terapötik (1), ameliyat öncesi de kompresyon (2) ve palyasyon (11) amacıyla gerçekleştirilmiştir. Kateterlerin kalış süresi 2-14 hafta olmuştur. İki hastada kolanjitis, iki hastada kısa süreli hemobiliya gelişmiş, üç hastada kateterin yerinden çıktığı saptanmıştır. Klatskin tümörü olan bir hastaya PTBD'dan altı hafta sonra stent konulmuştur. Son on yılda ameliyat öncesi biliyer drenajın yararı tartışmalı bir noktaya gelmesine rağmen, PTBD, proksimal safra yolu tutulumlarında ya da cerrahi veya stentleme için uygun aday olmayan ve yaklaşık üç ay yaşam beklentisi bulunan malin tıkanma sarılıklı hastalarda etkin bir palyasyon yöntemi olarak kullanılabilir.
Abstract
In recent years, there has been a great interest amongst the general surgeons in using of the interventional procedures for the evaluation and treatment of surgical disease. The authors report on the preliminary experience they have gained with regard to the techniques and management of percutaneous transhepatic biliary drainage (PTBD) as a non-operative method for providing biliary drainage in treating 13 patients with obstructive jaundice which of 12 were due to malignancy. In these patients fourteen PTBD procedures (8 were under fluoroscopic and 6 were under ultrasonographic guidance) were performed by the same surgeon. The interventions were accomplished for therapeutic (I), preoperative decompression (2) and for palliative (II) purposes. The duration of the catheters in use was 2 to 14 weeks. The complications due to PTBD consisted of short term hemobilia in two, cholangitis in two and catheter displacement in three patients. One of the patients who had a Klatskin tumor underwent intrabiliary stenting six weeks after PTBD. Although, in the last decade the benefits of preoperative biliary drainage has become conceptually cumbersome, PTBD may be used as the primary palliative procedure with ease in patients with a nearly three months life expectancy who are not eligable candidates for surgery or stenting and those with proximal biliary involvement.
