ARDS modelinde hidroksiüre ve metil prednizolonun karaciğer, böbrek ve barsak hasarının önlemede rolü (Deneysel çalşıma)

Özet

Adult respiratuar distres sendromunda (ARDS) önce akciğerlerde, sonra diğer yaşamsal organlarda biriken lökositlerden açığa çıkan enzim ve toksit maddeler kopiller permeabilite artışına, intemtisiyel ödeme ve doku harabiyetine neden olur. Lökosit sayısının azaltılması veya fonksiyonlarının inhibe edilmesi ile bu hasarın azaltılabileceği fikrinden hareketle deneysel ARDS modelinde hidroksiüre (HU) ve metilprednizolon (MP)'un karaciğer, böbrek ve terminal ileum dokularında oluşturduğu biyokimyasal ve histopatolojik değişikliklerin incelenmesi amaçlandı. 24 Adet New Zealand cinsi beyaz tavşan 6 şarlı 4 gruba ayrıldı. Denekler ketamin anestezisi altında Kontrol (K) grubunda medikasyonsuz diğer 3 grupta ise 0.12 mglkg dozunda oleik asit IV verildikten sonra ventilatöre bağlandı. Oleik Asit (OA) grubundaki deneklere başka işlem uygulanmadı. Oleik asit infüzyonundan 1 saat önce MP grubuna IV 30 mglkg MP, HU grubuna ise 20 gün süreyle 50 mglgün HU verildi. Lokosit sayılan OA, MP ve HU gruplarında 2. ve 4. saatlerde bir önceki ölçümlere ayrıca OA grubunda K grubuna, MP ve HU gruplarında OA grubuna oranla anlamlı derecede düşük idi. Karaciğer, böbrek ve terminal ileum dokularındaki PMNL, MNL ve AM sayısı OA grubunda K grubundan anlamlı derecede yüksek, MP ve HU gruplarında OA gruhundakinden anlamlı derecede düşük idi. Karaciğer, böbrek ve terminal ileum dokularındaki MDA seviyeleri O A grubunda K grubuna oranla anlamlı derecede yüksek iken, M P grubunda O A. grubundan anlamlı derecede düşük saptandı. HU grubunda, ise bu düşüş sadece böbrek ve ileum dokuları için anlamlı idi. Deneysel ARDS modelinde; MP lökositleri karaciğer, böbrek ve barsak dokusuna migrasyonunu önleyerek ve fagositoz yeteneğini inhibe ederek bu organlardaki lip id peroksidasyonunu ve doku hasarını azaltmaktadır. HU ise dolaşımdaki ve paralel olarak dokudaki lökositlerin sayısını azaltmasına rağmen sadece barsak ve böbrekteki hasarı azaltmaktadır.

Abstract

In adult respiratory distress syndrome (ARDS) leukocytes are collected initally in lung and than in other organs and release several enzymes and toxic materials that increase permeability in capillary and cause edema and damage in those tissues. An experimental study originated from the idea that damage could be decreased by reducing the amount and inhibiting the functions of leukocytes was planned. The efficiency of methylprednisolone (MP) and hydroxiurea (HU) on damage of liver, kidney and terminal ileum secondary to ARDS in that experimental model was investigated by biochemical and histopathological examinations. There were 4 groups each consisting of 6 New Zealand type white rabbits. After anaesthetized by ketamin, rabbits underwent mechanical ventilation; those in control (C) group without any medication but the rest after being applied IV 0.12 mglkg oleic acid. The Oleic 'Acid (OA) group had no other application. MP (30 mg/kg/lV) in MP group and HU (50 mg/day/per orally) for a 20 day period in HU group were applied. There was a significant difference between the number of leukocytes in O A, MP and HU groups at 2nd and 4th hours. That number was significantly lesser in OA group than C group, in MP and HU groups than O A group. PMNL, MNL and AM numbers in liver, kidney and terminal ileum were significantly higher in OA group than in C group while those numbers were lesser in MP and HU groups than in those of O A group. The levels of MDA in liver, kidney and terminal ileum were higher in OA group than in C group while they were lower in MP group than in OA group. In HU group, only the levels of MDA in kidney and terminal ileum were significantly lower than in OA group. MP reduces lipid peroxidation and tissue damage by preventing migration of leukocytes to liver, kidney and intestine and inhibiting their phagocytic activity. HU reduces the number of leukocytes both in circulation and tissues but could only improve the damage in kidney and ileum. Key words: ARDS, liver, kidney, ileum, methylprednisolone, hydroxurea
