Özet
Akut pankreatitte prognozun belirlenmesi amacıyla kullanılan Ranson ve APACHE II gibi multifaktöryal skorlama sistemlerinin klinik uygulaması güç olmaktadır. Fibronektinin pankreatitli olgularda hastalığın şiddetine paralel olarak yükseldiği konusunda çeşitli çalışmalar yapılmış ancak farklı sonuçlar elde edilmiştir. Bu çalışmada, Ocak 2000-Ocak 2001 tarihleri arasında Cerrahpaşa Tıp Fakültesi, Acil Biriminde akut pankreatit tanısı konulan 57 olguda, ilk 24 saatte kan örneği alınarak fibronektin ve akut faz reaktanlanna bakıldı. Bu olguların fibronektin ve akut faz reaktanlarının değerleri karşılaştırıldığında, hastalığın şiddeti ile akut faz reaktanlarından CRP'nin artışı arasında anlamlı ilişki bulundu; ferritin ve haptoglobin ile ilişkisi ise anlamsızdı. Fibronektin ise 2 ödematöz pankreatitli olgu dışında normal değerlerde idi. Sonuç olarak, C-reaktif protein, akut pankreatitin şiddetini değerlendirmede erken, kesin sonuç veren, kolay ulaşılabilir ve ucuz bir test olması nedeni ile tercih edilen bir parametre iken fibronektin, erken dönemde prognozun belirlenmesinde etkin bulunmamıştır.
Abstract
The clinical application of multifactorial scoring systems such as Ranson and APACHE II in the evaluation of acute pancreatitis is found to be difficult. Therefore, serum amylase and lipase levels, and acute phase reactants are widely used in clinical practice. There are several studies reporting a higher fibronectin level in severe forms of acute pancreatitis, but its value as a prognostic criteria is not certain. The aim of this study was to investigate the role of fibronectin in acute pancreatitis by comparing it with acute phase reactants like haptoglobulin, ferritin and Creactive protein (CRP) levels. 57 patients with acute pancreatitis applied to Department of Emergency Surgery of Cerrahpaşa School of Medicine, between January 2000 and January 2001. Blood samples were taken to detect the levels of fibronectin and acute phase reactants in the first 24 hours. The comparison of fibronectin and acute phase reactant levels correlated with the severity of disease revealed a significant relation for CRP; however, there was no relation with ferritin and haptoglobulin. Fibronectin levels were in normal range except in two cases. As a conclusion, Creactive protein should be preferred to evaluate the severity of acute pancreatitis, as it is a simple, cheap and widely employed test. Fibronectin is found to be ineffective in the evaluation of the severity of disease in early phase.
