Özet

Kolon anastomozlannın yaklaşık % 5'inde klinik olarak anastomoz kaçağı görülüyor olmasına rağmen, gerçek insidans muhtemelen çok daha fazladır. Bu istenmeyen komplikasyon belirgin morbidite ve mortaliteye yol açabilir. Bu çalışmada, siyanoakrilat uygulaması ve polipropilen sütür ile yapılan kolon anastomozlannın karşılaştırılması amaçlanmaktadır. Kırk adet WistarAlbino cinsi sıçan kullanıldı. Sıçanlar 20'şer sıçandan oluşan iki ana gruba ayrıldı. Birinci grupta kolon rezeksiyonu ve polipropilen sütiir anastomoz uygulandı. Daha sonra 10 sıçan (grup 1a) postoperatif üçüncü gün ve diğer 10 sıçan,(grup 1b) postoperatif yedinci gün anastomoz değerlendirilmesi için réopère edildi. İkinci grupta ise kolon, rezeksiyonu ve siyanoakrilat uygulaması ile anastomoz uygulandı. Daha sonra 10 sıçan (grup 2a) postoperatif üçüncü gün ve diğer 10 sıçan (grup 2b) postoperatif yedinci gün anastomoz değerlendirilmesi için réopère edildi. Anastomoz değerlendirmesi anastomoz pat/ama basıncı, doku hidroksiprolin seviyesi ve anastomoz hattı fibrosis yoğunluğu ölçülerek yapıldı. Abstract

Disruption of the suture line is clinically apparent in approximately 5 % of all colonic anastomoses, hut the true incidence is probably much higher. This devastating complication may lead to significant, morbidity and mortality. The aim of the study is to compare the effects of both cyanoacrylate application and the polypropylene suturing on colonic anastomosis. Forty Wistar Albino rats were used. The rats were separated into two main groups of 20 rats. In the first group, colonic resection with polypropylene suture anastomoses was applied. In this group, 10 of rats (group la) were reoperated on the third and the other 10 (group Ib) on the seventh day for anastomotic assessment. In the second group, colonic resection, with cyanoacrylate anastomoses was applied. In this group, 10 of rats (group 2a) were reoperated on the third and the other 10 (group 2b) on the seventh day for anastomotic assessment. At the reoperation anastomotic strength was assessed by bursting pressure of the anastomoses, tissue hydroxyproline level and thefibrotic density of the anastomotic site. While the bursting pressures in group 2a were significantly higher than those of group la on third day postoperatively (p0.05) and anastomotic collagen density. It is concluded that cyanoacrylate application for colonic anastomotic support can be done safely and it seems effective especially in early postoperative period. But more detailed and advanced experimental and clinical studies are needed to determine its safety and possible side effects.
