Multinodüler guatr'ın cerrahi tedavisinde tercih ne olmalı?

Özet
Kliniğimizde multinodüler guatr nedeniyle(MNG) ameliyat edilen olguları, ameliyat, tekniği ve bu tekniğe bağlı oluşan rekürren sinir paralizisi, hipoparatiroidizm ve gelişen nüksler açısından değerlendirmek amacıyla bu çalışma yapılmıştır. Ocak 1999-Mayıs 2002 tarihleri arasında MNG nedeniyle ameliyat edilen 80 olgu randomize şekilde iki gruba ayrıldı. Preoperatif dönemde dominant nodülden yapılan ince iğne aspirasyon biyopsisi (İİAB) ve operasyon esnasında frozen sectionda malign yada şüpheli sitolojik tanı alan olgular çalışma dışı bırakıldı. Grup I'deki 40 olguya bilateral subtotal tiroidektomi (BST), Grup II'deki 40 olguya ise unilateral total+kontralateral subtotal, tiroidektomi (UTKST) işlemi uygulandı. Ameliyat sonrası dönemde; Grup l'de 2 olguda, Grup II de ise 3 olguda geçici hipokalsemi ve Grup I de bir olguda, Grup II'de 3 olguda geçici vokal kordparalizisi saptandı. Ameliyattan 3 ay sonra yapılan tiroid fonksiyon testlerinde Grup l'de I olguda, Grup II'de ise 5 olguda hipotiriodiye rastlandı. Histopatolojik sonuçlara göre, Grup I de 2 (% 5) olguda dominant nodulün karşı tarafında, Grup II de ise 3 (% 7.5) olguda dominant nodül tarafında popüler tiroid karsinomuna rastlandı. Grup I deki olgulara reoperasyonla bilateral total tiroidektomi işlemi yapıldı. Grup I'deki olgulara reoperasyonda total tiroidektomi uygulandı. Grup l ve Grup II'deki olgularda, sırasıyla 18.6 ve 17.2 aylık sürede nükse rastlanmadı. Sonuç olarak; MNG'in cerrahi tedavisinde; dominant nodul tarafına total, karşı tarafa subtotal tiroidektomi tekniğini, BST ile aynı morbiditeye sahip olması ve ayrıca olgularda çıkabilecek olan malignitelerde reoperasyona ve buna bağlı gelişebilecek sinir ve paratiroid hasarına daha az neden olması bakımından önermekteyiz.
Abstract
This study was designed to decide to figure out the outcome of the patients whom were operated due to multinodular goiter (MNG) in our clinic for recurrent laryngeal nerve paralysis, hypoparathyroidism and recurrences. 80 patients whom were operated due to MNG between January 1999-May 2002 were randomly divided into two groups. The patients with diagnosis of thyroid carcinoma by fine needle aspiration biopsy or by frozen section were abandoned from this study. 40 patients in group I had bilateral subtotal thyroidectomies (BST) and 40 patients in group II had unilateral total plus contralateral subtotal thyroidectomies. In the early postoperative period two patients in group I and three patients in group 11 had transient hypocalcemia, and one patient in group I and three patients in group II had transient vocal cord paralysis. Thyroid function tests which were done 3 months after the operation revealed one patient in group I and five patients in group II having hypothyroidism. Histopathological results revealed two cases of papillary thyroid carcinoma which were at the contralateral site of the dominant nodule in group I, and three cases of papillary thyroid carcinoma which were located at the dominant nodule in group II. For cancer cases in group I, reoperation was performed as total thyroidectomy. In a followup period of 18.6 months and 17.2 months for group I and H respectively, there exists no recurrence. Surgical treatment of MNG by total thyroidectomy to the dominant nodule site plus contralateral subtotal thyroidectomy has the same morbidity as BST, and this procedure reduces the risk of nerve and parathyroid injury due to the reoperations for cancer existence.
