Özet

Amaç: Çürüğün yeterli temizlenmemesi sonucukavite içerisinde geride kalan bakteri miktarırestoratif diş hekimliğinde sorun oluşturmaktadır.Pulpa kaplaması veya kaide materyali uygulanımıöncesi kavite dezenfeksiyonu önerilmektedir. Buçalışmanın amacı ticari olarak piyasada yer alan jelformülasyonlarının total etch bir kompozit sisteminmikro-gerilme bağlanma dayanımı (MGBD)üzerinde oluşturduğu etkinin değerlendirilmesidir. Gereç ve Yöntem: Ticari olarak piyasadayeralan klorheksidin [(Corsdoyl gel- %1KHX- COG), (Cervitec gel-%0,2 KHX+%0,2 sodyumfluorid-CEG), aleo vera (Forever Bright-FOB) vehyaluronik asit (Gengigel-GEG) içerikli ticariantibakteriyel jel formülasyonları bu çalışmadakullanıldı. Kontrol grubunda ise (KON) herhangibir dezenfektan uygulanmadı. Çalışmada her birgrup için 2 adet olmak üzere toplam 10 adetçekilmiş çürüksüz insan 3.molar dişi kullanıldı. Jelgruplarında total etch sistem uygulamanın ardındanbonding materyalin kullanımından hemen önce 1dakika süre ile jeller uygulandı, steril peletleortamdan uzaklaştırıldı ve hava ile kurutuldu. Dahasonra kompozit restorasyonlar yapıldı. 24 saat süreile 370Cde distile suda saklanan örneklerden herbir grupta 10ar adet olacak şekilde (kesit çapı1,0±0,1 mm2) kompozit-dentin çubuk örneklerioluşturuldu. Bu örnekler daha sonra dakikada 1 mmkuvvet uygulanarak MGBD testine tabi tutuldu.Kompozit-dentin arası bağlanma dayanım değerlerimegapaskal cinsinden (MPa) kaydedildi ve kopmatipleri değerlendirildi. İstatistiksel farklılıklar tekyönlü ANOVA ve Tukeys testi ile pCOG>GEG>FOB (p>0,05). Kontrolgrubunda karışık kopma tipi baskın olarakgörülürken diğer tüm gruplarda adeziv kopma tipigörüldü. Sonuç: Bu tip jellerin antibakteriyelözelliklerinin yanı sıra kullanımları sırasındamikro-mekanik bağlantı ve klinik performanslarınaait değişiklikler göz ardı edilmemelidir.

Abstract

Objectives: Insufficient removal of caries- affected enamel and dentin represents a seriousproblem in restorative dentistry, as any bacteriaremaining in the cavity after preparation may causesecondary caries. In order to provide an asepticenvironment before pulp capping and lining, the useof an antibacterial material as a cavity disinfectanthas been recommended. The aim of this study wasto evaluate the role of commercially available gelformulations on the micro-tensile bond strength(MTBS) of a total etch system. Materials and Methods: Commerciallyavailable gel formulations with the contents ofchlorhexidine (Corsodyl gel- %1CHX-COG),(Cervitec gel-%0.2 CHX+%0.2 sodium fluoride- CEG), aloe vera (Forever Bright-FOB) andhyaluronic acid (Gengigel-GEG) applicable ascavity disinfectants were used. No disinfectant wasapplied in the control group (CON). Ten humanthird molar free of caries were used in the study.Gels were applied for 1 minute just before bondingfollowing total etch application in the gel groups,removed with sterile pellets and air-dried. Thencomposite restorations were applied. Tencomposite-dentin rods (surface area of 1.0±0.1mm2) were generated in each group from specimensstored in distilled water in 370C for 24 hours. Thesespecimens then were subjected to MTBS testapplying at a crosshead speed of 1 mm/min. Bondstrength values between composite-dentin wererecorded as megapascal and failure types wereevaluated. Statistical differences were evaluatedwith one-way ANOVA and Tukey s test at thesignifince level of pGEG>FOB (p>0.05). While mixedfailure type was dominant in the CON group, onlyadhesive failure type was observed in all the othergroups. Conclusions: Besides the antibacterialproperties of these gel formulations, alterationsoccured in micromechanical adhesion and clinicalperformances should not be overlooked.
